摘要
目的探讨神经肌电图检查在2型糖尿病患者无症状周围神经病变(diabetic peripheral neuropathy,DPN)诊断中的应用价值。方法选取2022年1月—2024年12月黔东南州人民医院收治的70例合并无症状DPN的2型糖尿病患者为研究样本,纳入DPN组,同期70例未合并DPN的2型糖尿病患者,纳入无DPN组,均进行神经肌电图检查,比较两组检查结果,分析神经肌电图检查在2型糖尿病患者无症状DPN诊断中的应用价值。结果DPN组正中神经、腓总神经运动神经传导速度(motor nerve conduction velocity,MCV)、感觉神经传导速度(sensory nerve conduction velocity,SCV)低于无DPN组,F波潜伏期长于无DPN组,差异均有统计学意义(P均<0.05)。绘制受试者工作(receiver operating characteristic,ROC)曲线显示,正中神经MCV、SCV、F波潜伏期、腓总神经MCV、SCV及F波潜伏期诊断无症状DPN的AUC分别为:0.912、0.873、0.832、0.906、0.768、0.875。结论神经肌电图检查能够敏感地检测2型糖尿病患者无症状DPN的早期神经功能异常,其中正中神经和腓总神经的MCV、SCV以及F波潜伏期是重要的诊断指标,可为临床提供可靠的诊断依据。
Objective To explore the application value of nerve electromyography in the diagnosis of asymptomatic diabetic peripheral neuropathy(DPN)in patients with type 2 diabetes.Methods A total of 70 patients with type 2 diabetes mellitus complicated with asymptomatic DPN admitted to Qiandongnan Prefecture People's Hospital from January 2022 to December 2024 were selected as the study samples and included in the DPN group.At the same time,70 patients with type 2 diabetes mellitus without DPN were included in the non-DPN group.All patients underwent nerve electromyography examination.The results of the two groups were compared,and the application value of nerve electromyography examination in the diagnosis of asymptomatic DPN in patients with type 2 diabetes mellitus was analyzed.Results The motor nerve conduction velocity(MCV)and sensory nerve conduction velocity(SCV)of median nerve and common peroneal nerve in DPN group were lower than those in non-DPN group,and the latency of F wave was longer than that in non-DPN group,the differences were statistically significant(all P<0.05).The receiver operating characteristic(ROC)curve showed that the AUC of median nerve MCV,SCV,F wave latency,common peroneal nerve MCV,SCV and F wave latency in the diagnosis of asymptomatic DPN were 0.912,0.873,0.832,0.906,0.768 and 0.875,respectively.Conclusion Nerve electromyography can sensitively detect the early neurological dysfunction of asymptomatic DPN in patients with type 2 diabetes mellitus.MCV,SCV and F wave latency of median nerve and common peroneal nerve are important diagnostic indicators,which can provide reliable diagnostic basis for clinical practice.
