摘要
目的:原发性睾丸弥漫大B细胞淋巴瘤(PT-DLBCL)作为一种罕见疾病,其治疗方案在国内外尚未形成统一标准。文章旨在通过报道1例患者的治疗过程,探讨诊疗方案的临床效果,为今后的临床实践提供参考。方法:回顾性分析2023年8月在联勤保障部队第945医院接受治疗的1例PT-DLBCL患者的临床资料、诊断、治疗及随访过程。结果:患者因阴囊肿大伴疼痛就诊,经腹股沟右侧睾丸根治性切除术后,病理诊断为PT-DLBCL。随后采用R-CHOP方案化疗,并行健侧睾丸预防性放疗。术后8个月回访显示,患者呈无进展生存状态。结论:PT-DLBCL临床表现无特异性,术前诊断困难。由于该病具有高度侵袭性和预后差的特点,早期诊断和治疗至关重要。根治性睾丸切除术联合放化疗可降低复发概率,明显改善预后,提高总生存期。因此,对于疑似病例,应尽早进行详细的检查和评估,以制定个体化的治疗方案。文章为PT-DLBCL的诊疗提供了有益的参考,但鉴于该病的罕见性,仍需进一步积累病例,开展多中心研究,以形成更为完善的治疗方案。
Objective:Currently,there is a paucity of standardized therapeutic approaches for primary testicular diffuse large B-cell lymphoma(PTL-DLBCL)globally,primarily due to its rarity and limited retrospective studies.To report 1 case report on the management of PTL-DLBCL,and discuss its diagnostic and therapeutic outcomes.Methods:The clinical data of 1 patient with primary testicular diffuse large B-cell lymphoma who was treated at the 945th Hospital of the Joint Logistic Support Force in August 2023 were retrospectively analyzed.The diagnosis,treatment and follow-up process were elaborated in detail.Results:The patient presented with scrotal swelling accompanied by pain.After radical resection of the right testicle in the inguinal region,the pathological diagnosis was primary diffuse large B-cell lymphoma of the testis.Subsequently,chemotherapy with the R-CHOP regimen was adopted,and prophylactic radiotherapy was performed on the healthy testicle.The follow-up visit 8 months after the operation showed that the patient was in a progression-free survival state.Conclusion:While the clinical presentation of this case is nonspecific,leading to diagnostic challenges preoperatively,the high aggressiveness and poor prognostic implications of PTL-DLBCL necessitate prompt attention.Early diagnosis and prompt intervention,comprising radical orchiectomy combined with radiotherapy and chemotherapy,can significantly reduce the risk of recurrence,enhance prognosis,and prolong overall lifetime.Retrospective analysis of this case underscores the efficacy of a multimodal approach encompassing surgical resection,radiotherapy,and chemotherapy in the management of PTL-DLBCL.This case report provides useful reference for the diagnosis and treatment of primary testicular diffuse large B-cell lymphoma.However,in view of the rarity of this disease,further case accumulation and multi-center studies are still needed to form a more complete treatment plan.
