摘要
目的:探讨精子DNA完整性对体外受精-胚胎移植技术(IVF-ET)不孕症夫妇胚胎发育及临床结局的影响。方法:选取2022年1月—2024年1月于焦作市妇幼保健院接受IVF-ET的286对不孕症夫妇作为研究对象。常规检测分析精液,计算精子DNA碎片指数(DFI),并根据精子DFI将研究对象分为高DFI组(DFI>30%)和低DFI组(DFI≤30%)。比较两组患者胚胎发育情况及临床结局,分析精子DNA完整性对IVF-ET不孕症夫妇胚胎发育及临床结局的影响。结果:研究共286对不孕症夫妇,其中高DFI组共83对夫妇,低DFI组共203对夫妇。高DFI组夫妇受精率、优质胚胎率、种植率、妊娠率均低于低DFI组,差异均有统计学意义(χ^(2)=4.944、6.865、6.173、7.225,P<0.05)。logistic回归分析结果显示,精子DNA完整性对IVF-ET不孕症夫妇胚胎发育及临床结局中的受精率、优质胚胎率、种植率、妊娠率有影响(OR=1.905、2.008、2.134、2.024,P<0.05)。结论:精子DNA完整性对IVF-ET不孕症夫妇胚胎发育及临床结局具有重要影响,男方DFI高的不孕症夫妇在受精率、优质胚胎率、种植率和妊娠率方面表现出明显劣势。精子DNA完整性水平可能是一个重要的预测因素,对于IVF-ET的成功率和临床结局具有重要意义。
Objective:To explore the impact of sperm DNA integrity on embryonic development and clinical outcomes in infertile couples undergoing in vitro fertilization embryo transfer(IVF-ET)technology.Methods:286 infertile couples who underwent IVF-ET in hospitals from January 2022 to January 2024 were selected as the study subjects.Semen was routine tested and analyzed,and sperm DNA fragmentation index(DFI)was calculated.The study subjects were divided into high DFI group(DFI>30%)and low DFI group(DFI≤30%)based on sperm DFI.The embryonic development and clinical outcomes of two groups were compared,and the impact of sperm DNA integrity on the embryonic development and clinical outcomes of infertile couples undergoing in vitro fertilization embryo transfer were analyzed.Results:In this study,286 infertile couples were divided into a high DFI group of 83 couples and a low DFI group of 203 couples based on sperm DFI.The fertilization rate,high-quality embryo rate,implantation rate and pregnancy rate of couples in the high DFI group were all lower than those in the low DFI group,and the differences were statistically significant(χ^(2)=4.944,6.865,6.173,7.225;P<0.05).The results of logistic regression analysis showed that the integrity of sperm DNA had an impact on the embryo development and clinical outcomes of infertile couples undergoing IVF-ET,including the fertilization rate,high-quality embryo rate,implantation rate,and pregnancy rate(OR=1.905,2.008,2.134,2.204;P<0.05).Conclusion:The integrity of sperm DNA has significant impact on the embryonic development and clinical outcomes of infertile couples undergoing in vitro fertilization embryo transfer.Infertile couples with high male DFI exhibit significant disadvantages in terms of fertilization rate,high-quality embryo rate,implantation rate,and pregnancy rate.Therefore,the level of sperm DNA integrity may be an important predictive factor for the success rate and clinical outcomes of IVF-ET.
