摘要
目的观察清肺排毒汤配合激素治疗对免疫检查点抑制剂相关性肺炎(CIP)的应用效果。方法本研究为前瞻性研究,研究对象为河南大学第一附属医院2022年1月至2023年9月期间收治的120例肺癌CIP患者,应用计算机生成随机序列并进行分配隐藏后,将其分别列为常规组(60例)和中药组(60例)。常规组实施常规激素治疗,中药组采用清肺排毒汤配合激素治疗,治疗结束后随访一年,比较两组患者的炎症指标、免疫功能、临床-影像-生理(CRP)评分以及生存质量。结果在不同治疗方案下,中药组的肿瘤坏死因子-α(TNF-α)、白细胞介素-1β(IL-1β)、白细胞介素-6(IL-6)、白细胞介素-8(IL-8)均低于常规组,差异有统计学意义(P<0.05);中药组的T淋巴细胞亚群CD3^(+)、CD4^(+)、CD4^(+)/CD8^(+)均高于常规组,CD8^(+)低于常规组,差异有统计学意义(P<0.05);中药组的CRP评分中临床得分、影像得分及生理得分均低于常规组,差异有统计学意义(P<0.05)。截至随访结束时,中药组的一年内生存率、功能状态评分(KPS)均高于常规组,美国东部肿瘤协作组体能状况量表(ECOG-PS)评分低于常规组,差异有统计学意义(P<0.05)。结论清肺排毒汤配合激素治疗能减轻CIP患者的炎症反应及免疫功能,对促进患者症状恢复并提升短期生存质量均有积极意义。
Objective Observation of the application effect of qingfei paidu decoction combined with hormone therapy on immune checkpoint inhibitor associated pneumonia(CIP).Methods This study was a prospective study,with 120 lung cancer CIP patients admitted to First Affiliated Hospital of Henan University from January 2022 to September 2023.A computergenerated random sequence was assigned and hidden,and 120 CIP patients were divided into a conventional group(60 cases)and a traditional Chinese medicine group(60 cases).The conventional group received conventional hormone therapy,while the traditional Chinese medicine group received qingfei paidu decoction combined with hormone therapy.After treatment,the two groups were followed up for one year.Compare inflammatory indicators,immune function,CRP scores,and quality of life between two groups.Results Under different treatment plans,the levels of tumor necrosis factor-α(TNF-α),interleukin-1β(IL-1β),interleukin-6(IL-6),and interleukin-8(IL-8)in the traditional Chinese medicine group were lower than those in the control group(P<0.05).The T lymphocyte subsets CD3^(+),CD4^(+),CD4^(+)/CD8^(+)in the traditional Chinese medicine group were higher than those in the control group,while CD8^(+)was lower than that in the control group(P<0.05).The clinical scores,imaging scores,and physiological scores of CRP in the traditional Chinese medicine group were lower than those in the control group(P<0.05).As of the end of follow-up,the one-year survival rate and functional status scores(KPS)of the traditional Chinese medicine group were higher than those of the conventional group,while the Eastern cancer collaboration group in the United States(ECOG-PS)scores in the traditional Chinese medicine group were lower than those of the conventional group(P<0.05).Conclusion The combination of qingfei paidu decoction and hormone therapy can alleviate the inflammatory response and immune function of CIP patients,and has a positive effect on promoting symptom recovery and improving short-term quality of life.
