摘要
目的:探讨内镜下喷洒去氨加压素联合钛夹在消化道息肉切除患者中的应用效果及对创面愈合的影响。方法:选择2023年2月~2024年2月安福县人民医院消化道息肉切除术患者72例为研究对象,根据治疗方法不同分为两组各36例。对照组给予钛夹干预,观察组联合内镜下喷洒去氨加压素治疗,两组治疗7d后评估患者效果,并对患者完成6个月随访,比较两组凝血功能、创面愈合、住院、术后禁食、术后下床活动时间、术后并发症。结果:两组干预后凝血功能得到改善;观察组PT、APTT、TT、FIB及D-D低于对照组(P<0.05);观察组创面愈合、住院、术后禁食及术后下床活动时间短于对照组(P<0.05);两组术后并发症比较无统计学差异(P>0.05),两组术后2个月及4个月复发率无统计学差异(P>0.05);观察组6个月复发率低于对照组(P<0.05)。结论:内镜下喷洒去氨加压素联合钛夹用于消化道息肉切除术患者,有助于改善凝血功能,缩短创面愈合及术后恢复时间,未增加术后并发症发生率。
Objective:To investigate the application effect of endoscopic spraying of desmopressin combined with titanium clips in patients undergoing gastrointestinal polypectomy and its impact on wound healing.Methods:72 patients who underwent gastrointestinal polypectomy at Anfu County People’s Hospital from February 2023 to February 2024 were selected as the subjects and divided into two groups of 36 patients each according to different treatment methods.The control group was given titanium clip intervention,while the observation group was treated with endoscopic spraying of desmopressin.After 7 days of treatment,the effectiveness of the two groups was evaluated,and patients were followed up for 6 months.The coagulation function,wound healing,hospitalization,postoperative fasting,postoperative mobilization time,and postoperative complications were compared between the two groups.Results:After intervention,coagulation function was improved in both groups;The PT,APTT,TT,FIB,and D-D levels in the observation group were lower than those in the control group(P<0.05);The observation group had shorter wound healing,hospitalization,postoperative fasting,and postoperative mobilization time compared to the control group(P<0.05);There was no statistical difference in postoperative complications between the two groups(P>0.05),and there was no statistical difference in recurrence rates at 2 and 4 months after surgery between the two groups(P>0.05);The 6-month recurrence rate of the observation group was lower than that of the control group(P<0.05).Conclusion:Endoscopic spraying of desmopressin combined with titanium clips can improve coagulation function,shorten wound healing and postoperative recovery time in patients undergoing gastrointestinal polypectomy,without increasing the incidence of postoperative complications.
