牙槽突裂植骨前后上颌矫形力对牙槽突位移影响的有限元分析

Effect of Maxillary Orthodontic Force on Alveolar Displacement before and after Alveolar Cleft Bone Grafting:A Finite Element Analysis

摘要 目的:模拟比较牙槽突裂植骨前后上颌矫形治疗对牙槽骨三维方向位移的影响。方法:在原唇腭裂上颌骨有限元模型上,模拟牙槽突裂植骨,形成牙槽突裂植骨前后有限元模型,在2个模型上分别添加相同上颌矫形力,观察牙槽突位移情况。结果:X轴、Y轴及Z轴位移云图结果显示:2个模型上颌牙槽骨区域均呈现位移增加趋势;植骨前模型,X轴方向位移量明显大于Y轴及Z轴(P<0.01),植骨后模型,Z轴方向位移量明显大于X轴及Y轴(P<0.01)。结论:牙槽突裂植骨前后行上颌水平向联合矢状向矫形治疗均能使牙槽突三维方向位移增加,植骨前行该治疗,应适当增加矢状向矫治力;植骨后行该治疗,应适当增加水平向矫治力。 Objective To simulate and compare the effect of maxillary orthodontic treatment on three-dimensional displacement of alveolar bone before and after alveolar cleft bone grafting.Methods On the finite element model of the original maxilla with cleft lip and palate,alveolar process bone grafting was simulated to form finite element models before and after alveolar process bone grafting.The same maxillary orthodontic force was added to both models,and the displacements of the alveolar process were observed.Results The displacement nephograms of the X-axis,Y-axis,and Z-axis showed an increasing trend of displacement in the maxillary alveolar bone area of both models;In the pre graft model,the displacement in the X-axis direction was significantly greater than that in the Y-axis and Z-axis(P<0.01),while in the post graft model,the displacement in the Z-axis direction was significantly greater than that in the X-axis and Y-axis(P<0.01).Conclusion Combination of maxillary horizontal with sagittal orthodontic treatment before and after bone grafting can increase displacement of the alveolar process in the three-dimensional direction.Before bone grafting,the sagittal orthodontic force should be appropriately increased,and after bone grafting,the horizontal orthodontic force should be appropriately increased.
