摘要
目的了解首都医科大学北京朝阳医院Xpert平台检测结核分枝杆菌(Mycobacterium tuberculosis,MTB)/利福平(Rifampicin,RIF)(Xpert MTB/RIF)应用情况。比较Xpert MTB/RIF检测与抗酸染色的阳性率和一致性。方法回顾性收集首都医科大学附属北京朝阳医院自2016—2023年的临床Xpert MTB/RIF筛查结果,统计Xpert MTB/RIF检测阳性标本的科室来源、类型分布及rpoB基因的突变特点。同时对Xpert MTB/RIF检测与抗酸染色法进行阳性率进行比较。结果2016—2023年首都医科大学附属北京朝阳医院Xpert MTB/RIF检测阳性率为3.56%,大体呈逐年下降的趋势;阳性标本主要来自于呼吸科病区,多为下呼吸道标本;利福平耐药基因检出率是6.35%,以探针E突变为主。Xpert MTB/RIF检出阳性率均高于抗酸染色法(P均<0.05),2种方法有较强的一致性,k=0.973。结论与抗酸染色相比,Xpert MTB/RIF检测的阳性率大幅提升,对于临床诊断结核病具有更好的辅助诊断作用。
Objective To investigate the application profile of the Xpert MTB/RIF at Beijing Chao-Yang Hospital,Capital Medical University,and to compare the positivity rates and concordance between Xpert MTB/RIF and acid-fast staining.Methods A retrospective analysis was conducted on clinical Xpert MTB/RIF results at Beijing Chao-Yang Hospital from 2016 to 2023.Data on departmental sources,specimen type distribution,and rpoB gene mutation characteristics of positive samples were collected.Positivity rates between Xpert MTB/RIF and acid-fast staining were compared.Results The positive rate of Xpert MTB/RIF testing at Beijing Chaoyang Hospital Affiliated to Capital Medical University from 2016 to 2023 was 3.56%,showing an overall declining trend year by year.Most positive specimens came from respiratory medicine wards,primarily lower respiratory tract samples.The detection rate of rifampicin-resistant genes was 6.35%,with Probe E mutations being the predominant type.The positivity rate of Xpert MTB/RIF was significantly higher than that of acid-fast staining(P<0.05),with substantial agreement between the two methods(k=0.973).Conclusion Compared to acid-fast staining,Xpert MTB/RIF significantly improves the detection rate of MTB and offers superior complementary diagnostic value for the clinical diagnosis of tuberculosis.
