干细胞对遗传相关视网膜疾病治疗效果的meta分析

摘要 目的运用meta分析评估干细胞对遗传相关视网膜疾病治疗的有效性。方法应用计算机在PubMed、Embase、Cochrane Library、Web of Science和CNKI上全面搜索2024年12月以前关于干细胞治疗遗传相关视网膜疾病的临床试验。采用MINORS偏倚风险评估工具评估纳入研究文献的质量,并采用数据分析软件进行数据分析。结果共纳入7项非随机对照试验,研究病例为115例。meta分析结果显示,在接受干细胞治疗的患者中,通过最小分辨率角的对数测量的最佳矫正视力有显著改善[MD=0.19,95%CI(0.07,0.31),P=0.002]。此外,与基线相比,多焦视网膜电图的反应性有所增强[MD=15.23,95%CI(-14.21,44.66),P=0.31]。视觉诱发电位振幅[MD=-0.26,95%CI(-1.43,0.91),P=0.66]和隐式时间变化[MD=5.39,95%CI(-4.41,15.19),P=0.28]在治疗后没有恶化。结论干细胞可以有效地治疗遗传相关视网膜疾病。 Objective To evaluate the efficacy of stem cell therapy in treating genetically related retinal diseases through a meta-analysis.Methods A comprehensive search was conducted using PubMed,Embase,Cochrane Library,Web of Science,and CNKI for clinical trials published up to December 2024 on stem cell therapy for genetically related retinal diseases.The MINORS tool was used to assess the risk of bias in the included studies,and data analysis was performed using statistical software.Results Seven non-randomized trials involving 115 participants were included.The meta-analysis revealed a significant improvement in best-corrected visual acuity(BCVA),measured by the logarithm of the minimum angle of resolution(logMAR),in patients receiving stem cell therapy(MD=0.19,95%CI[0.07,0.31],P=0.002).Additionally,multifocal electroretinogram(mfERG)responses showed enhancement compared to baseline(MD=15.23,95%CI[-14.21,44.66],P=0.31).Visual evoked potential(VEP)amplitude(MD=-0.26,95%CI[-1.43,0.91],P=0.66)and implicit time changes(MD=5.39,95%CI[-4.41,15.19],P=0.28)did not worsen post-treatment.Conclusion Stem cell therapy is effective in treating genetically related retinal diseases,demonstrating significant improvements in visual function without adverse effects on electrophysiological parameters.
