摘要
目的探讨分析中央坏死性乳腺癌患者的临床发病机制、表现和病理特征。方法选取甘肃医学院附属医院2019年1月-2024年2月收治的中央坏死性乳腺癌患者36例,均开展常规病理检查、免疫组化方法,观察临床表现、病理特征。结果病灶位于左侧乳房26例,右侧10例;边界模糊者13例,较清晰者23例。术前乳腺钼靶BI-RADS分级诊断依次为4B、4C、4C-5级。中央坏死性乳腺癌患者发生单个结节者27例,双结节5例,4例存在分叶状结节,结节中大部分边界清楚,中央均存在囊性改变/坏死的状况。PR、ER、HER-2在肿瘤内的阳性表达率为22.22%、36.11%、22.22%。中央坏死性乳腺癌中存在基底细胞样标记物的患者有26例。结论中央坏死性乳腺癌属于存在独特结构特征的浸润性癌,了解肿瘤的发病机制、临床表现和病理特征能预防漏误诊。
Objective To explore and analyze the clinical pathogenesis,manifestations,and pathological characteristics of central necrotic breast cancer patients.Methods Thirty six patients with central necrotic breast cancer from January 2019 to February 2024 in the Affiliated Hospital of Gansu Medical College were selected,and routine pathological examination and immunohistochemical methods were carried out to observe the clinical manifestations and pathological characteristics.Results Physical examination showed that the lesions were located in 26 cases on the left breast,10 cases on the right breast,13 cases with blurred boundaries,and 23 cases with clear boundaries.The preoperative breast molybdenum target BI-RADS grading diagnosis is 4B,4C,and 4C-5 grades,respectively.In central necrotic breast cancer,27 patients had a single nodule,5 patients had double nodules,and 4 patients had lobulated nodules.Most of the nodules had clear boundaries,and cystic changes/necrosis were found in the center.The positive expression rates of PR,ER,and HER-2 in tumors were 22.22%,36.11%,22.22%,respectively.26 cases of central necrotic breast cancer had basal cell like markers.Conclusion Central necrotic breast cancer is an invasive cancer with unique structural characteristics.Understanding the pathogenesis,clinical manifestations and pathological characteristics of the tumor can prevent misdiagnosis.
