摘要
目的 比较完全腹腔镜左半结肠癌根治术中行重叠式与传统三角吻合消化道重建的临床效果。方法 回顾性分析142例左半结肠癌患者资料,所有患者均行完全腹腔镜左半结肠癌根治术,根据消化道重建方式分为传统三角组(采用传统三角吻合消化道重建技术)与重叠式三角组(采用重叠式三角吻合消化道重建)。比较两组手术相关指标、肠道功能恢复指标、术后并发症发生情况及随访结果。结果 重叠式三角组消化道重建时间较传统三角组短(P<0.05);重叠式三角组术后首次排气时间、首次排便时间短于传统三角组(P<0.05);两组均无肿瘤局部复发及远处转移出现。结论 完全腹腔镜左半结肠癌根治术中应用传统与重叠式三角吻合消化道重建的安全性及预后效果相当,但后者可缩短消化道重建时间,促进患者术后肠道功能恢复。
Objective To compare the clinical efficacy of overlapping and traditional triangular anastomosis for gastrointestinal reconstruction in complete laparoscopic radical surgery for left colon cancer.Methods A retrospective analysis was conducted on the data of 142 patients with left colon cancer.All patients underwent complete laparoscopic left colon cancer radical surgery and were divided into the traditional triangle group(using traditional triangle anastomosis digestive tract reconstruction technique)and the overlapping triangle group(using overlapping triangle anastomosis digestive tract reconstruction)based on the digestive tract reconstruction method.The surgical related indicators,intestinal function recovery indicators,incidence of postoperative complications,and follow-up results were compared between the 2 groups of patients.Results The reconstruction time of the digestive tract in the overlapping triangle group was shorter than that in the traditional triangle group(P<0.05).The first exhaust time and first defecation time of the overlapping triangle group after surgery were shorter than those of the traditional triangle group(P<0.05).During the follow-up period,there was no local recurrence or distant metastasis of tumors in both groups.Conclusion The safety and prognosis of traditional and overlapping triangular anastomosis digestive tract reconstruction in complete laparoscopic left colon cancer radical surgery are comparable,but the latter can shorten the digestive tract reconstruction time and promote postoperative intestinal function recovery in patients.
