摘要
目的:评价益气培元颗粒治疗脾肾气虚型慢性肾脏病3~4期的临床疗效。方法:采用回顾性分析,入选2022年1月—2023年6月期间上海交通大学医学院附属仁济医院肾内科、中医科门诊患者75例,符合慢性肾脏病3~4期且脾肾气虚的诊断,根据治疗方案不同,分为对照组(CTL组)、益气培元组(YQPY组),两组患者均降压、降脂、纠正电解质紊乱等治疗。YQPY组在此基础上联合益气培元颗粒治疗6个月。对比治疗前后中医证候评分、血肌酐(serum creatinine, Scr)、肾小球滤过率(glomerular filtraton rate, eGFR)、24 h尿蛋白定量(24 h urine protein, 24 h Upro)、谷丙转氨酶(alanina aminotransferase, ALT)、谷草转氨酶(asparrate aminotransferase, AST)以及不良反应。结果:两组患者治疗后,YQPY组中医证候评分较CTL组低(P<0.01)。YQPY组患者24 h Upro、Scr均较治疗前明显下降(P<0.01,P<0.05),eGFR治疗后有提高(P<0.05),治疗组均优于对照组(P<0.05)。两组治疗前后ALT、AST差异均无统计学意义(P>0.05)。结论:对于脾肾气虚型慢性肾脏病3~4期患者,益气培元颗粒减少蛋白尿、降低Scr、提高eGFR,延缓肾功能恶化,且用药安全。
Objective:To evaluate the efficacy of Yiqipeiyuan Granule for the treatment of spleen-kidney qi deficiency type chronic kidney disease stage 3~4.Methods:Using retrospective analysis,75 CKD patients chronic kidney disease in stage 3~4 with spleen-kidney qi deficiency admitted to the outpatient clinics of the Department of Nephrology and TCM of Renji Hospital during the period of 2022.1-2023.6 were selected.And they were divided into CTL group and YQPY group according to dfferent treatment protocols.The changes of Chinese Traditional Medicine symptom score,blood creatinine,glomerular filtration rate,24 h urine pro tein,alanine aminotransferase,glutamine aminotransferase and adverse reactions were examed.Results:A statistically significant difference was observed between the CTL and YQPY groups after the treatment(P<0.01)in TCM symptom score.There was a sig-nificant difference in both groups before and after treatment of 24 h Upro and Scr(P<0.01,P<0.05),and the eGFR of YQPY group was better than the control group(P<0.05).The two groups did not difer significantly in terms of glutamine transaminase and azelaic transaminase before and after treatment(P>0.05).Conclusion:Yiqipeiyuan Granule can reduce urinary protein,lower blood creatinine,increase glomerular filtration rate,and delay the deterioration of renal function in chronic kidney disease stage 3~4 with spleen-kidney qi deficiency,and it is safe.
