摘要
目的分析乌司他丁、生长抑素联合血必净治疗急性胰腺炎(AP)的效果及对患者血淀粉酶(SAMY)、尿淀粉酶(UAMY)水平的影响。方法选取2021年1月—2023年12月南京市溧水区人民医院收治的98例AP患者,按照随机数字表法分为对照组(n=48,乌司他丁+生长抑素治疗)和观察组(n=50,乌司他丁+生长抑素+血必净治疗)。连续治疗10 d后,比较两组的症状缓解时间、临床疗效、不良反应及治疗前后的生化指标[肝素结合蛋白(HBP)、C反应蛋白(CRP)、血淀粉酶(SAMY)、尿淀粉酶(UAMY)、急性生理学与慢性健康状况评分Ⅱ(APACHEⅡ)]。结果与对照组相比,观察组的总有效率(98.00%)更高(χ^(2)=7.499,P=0.006),腹痛缓解时间、肠鸣音恢复时间、首次排便时间更短(t=8.499、3.565、5.440,P=0.000、0.001、0.000),HBP、CRP、AMS、UAMY水平及APACHEⅡ评分更低(t=8.297、13.075、13.396、19.457、7.122,均P=0.000)。观察组不良反应发生率为4.17%,低于对照组的10.00%,但组间差异无统计学意义(P>0.05)。结论乌司他丁、生长抑素联合血必净更为安全有效,可缩短症状缓解时间,减轻炎症反应及病情程度,降低SAMY、UAMY水平。
Objective To study the effect of ulinastatin,somatostatin combined with Xuebijing therapy on serum amylase(SAMY)and urine amylase(UAMY)levels in patients with acute pancreatitis.Methods A total of 98 AP patients who admitted to Lishui District People's Hospital of Nanjing from January 2021 to December 2023 were conducted on this study.All patients were divided into control group(n=48,Ulinastatin+Somatostatin treatments)and observation group(n=50,Ulinastatin+Somatostatin+Xuebijing treatments).After 10 days of continuous treatment,symptoms remission time,clinical efficacy,adverse reactions,biochemical indexes[heparin-binding protein(HBP)、Creactive protein(CRP)、serum amylase(SAMY)、urine amylase(UAMY)、acute physiology and chronic health evaluationⅡ(APACHEⅡ)].Results Compared with the control group,the total response rate was higher in the observation group(χ^(2)=7.499,P<0.05);Time to relief of abdominal pain,recovery of bowel sounds,and first bowel movement were shorter(t=8.499,3.565,5.440,all P<0.05);HBP,CRP,AMS,UAMY levels and APACHEⅡscore were lower(t=8.297,13.075,13.395,19.457,7.122,all P<0.05);the incidence of adverse reactions in the observation group was 4.17%,which was lower than 10.00%in the control group,but there was no significant difference between the two groups(P>0.05).Conclusion The combination treatment of Ulinastatin,Somatostatin and Xuebijinghas better effectson patients with acute pancreatitis in our small group study.
