摘要
目的:研究sMRI、fMRI及dMRI影像特征与感音神经性耳聋患者小脑功能的关系。方法:本研究为前瞻性研究,选取2023年4月—2024年4月于本院进行治疗的72例感音神经性耳聋患者作为研究对象,另选取同期进行健康体检的志愿者36例作为对照组,所有患者均行常规序列、DTI序列及BOLD序列MRI扫描。比较研究组以及对照组、不同小脑功能患者的sMRI脑灰质结果、dMRI脑白质结果以及fMRI结果之间的差异,研究sMRI、fMRI及dMRI影像特征与感音神经性耳聋患者小脑功能的关系。结果:研究组、小脑共济失调组患者的右侧颞叶梭状回以及右枕中回体素大小、MNI-X、MNI-Y、MNI-Z,右侧大脑脚、右侧丘脑前辐射、右侧内囊后肢、右侧皮质脊髓束、右侧外囊、胼胝体膝部、胼胝体压部、胼胝体体部的FA、AD、RD值均低于对照组、小脑功能正常组;研究组、小脑共济失调组患者的右侧颞下回、左侧颞下回、左侧额叶直回、左侧颞叶海马、右侧颞上回体素大小、MNI-X、MNI-Y、MNI-Z显著高于对照组、小脑功能正常组(P<0.05);小脑共济失调与s MRI及dMRI各项指标呈负相关,与fMRI各项指标呈正相关。结论:sMRI能够显示小脑的结构变化,fMRI可揭示小脑在听觉任务中的功能激活模式,而d MRI则通过检测小脑与其他脑区连接的完整性来评估其功能连接的变化。
Objective:This study aims to explore the relationship between sMRI,fMRI and dMRI imaging features and cerebellar functions in patients with sensorineural hearing loss.Methods:This prospective study was conducted from April 2023 to April 2024 and included 72 patients with sensorineural hearing loss and 36 volunteers who underwent health check-ups as the control group.All subjects were scanned using conventional sequence,DTI,and BOLD sequence.The study compared sMRI brain gray matter results,dMRI brain white matter results,and fMRI results between the study and control groups,as well as among patients with different cerebellar functions.We also investigated the relationship between sMRI,fMRI,dMRI imaging characteristics and cerebellar functions in patients with sensorineural hearing loss.Results:In the research and cerebellar ataxia groups,the FA,AD and RD values in specific brain regions(right temporal fusiform,right middle occipital gyrus,right cerebral peduncle,right thalamic radiation,right posterior limb of internal capsule,right corticospinal tract,right external capsule,genu,splenium,and body of the corpus callosum)were lower than in the control and normal cerebellar functions groups.The voxel size,MNI-X,MNI-Y,and MNI-Z values in other regions(right inferior temporal gyrus,left inferior temporal gyrus,left straight gyrus of frontal lobe,left hippocampus of temporal lobe,right superior temporal gyrus)were significantly higher in the research and cerebellar ataxia groups than in the control and normal cerebellar functions groups(P<0.05).Cerebellar ataxia showed negative correlations with sMRI and dMRI indicators but positive correlations with fMRI indicators.Conclusion:sMRI was able to show structural changes in the cerebellum,fMRI revealed patterns of functional activation in the cerebellum during auditory tasks,and dMRI assessed changes in the cerebellum's functional connections by detecting the integrity of its connections with other brain areas.
