摘要
目的:探讨蛛网膜下腔阻滞与全身麻醉在硬膜外穿刺失败后脑脊液漏患者中的应用效果。方法:选取2022年1月—2023年1月中国人民解放军第32183部队医院收治的硬膜外穿刺失败后伴脑脊液漏的患者60例,采用随机数字表法分为试验组(30例,蛛网膜下腔阻滞)和对照组(30例,全身麻醉)。对比两组术后疼痛程度、头痛发生率、恢复情况、并发症发生率。结果:试验组术后6、24、48 h的视觉模拟评分法评分和术后头痛发生率低于对照组(P<0.05)。试验组术后苏醒时间、住院时间均短于对照组(P<0.001),并发症发生率低于对照组(P<0.001)。结论:对于硬膜外穿刺失败后脑脊液漏患者,采用蛛网膜下腔阻滞作为补救麻醉方式的效果优于全身麻醉,术后镇痛效果较好,头痛发生风险较低,且恢复速度更快,并发症发生率更低。
Objective:To explore the application effect of subarachnoid block and general anesthesia in patients with cerebrospinal fluid leakage after failed epidural puncture.Methods:Sixty patients with cerebrospinal fluid leakage after failed epidural puncture who were admitted to the Hospital of the 32183rd Unit of the People's Liberation Army from January 2022 to January 2023 were selected and divided into the experimental group(30 cases,subarachnoid block)and the control group(30 cases,general anesthesia)by the random number table method.The degree of postoperative pain,the incidence of headache,the recovery situation and the incidence of complications were compared between the two groups.Results:The visual analogue scale scores at 6,24 and 48 hours after surgery and the incidence of postoperative headache in the experimental group were lower than those in the control group(P<0.05).The postoperative recovery time and hospital stay of the experimental group were shorter than those of the control group(P<0.001),and the incidence of complications was lower than that of the control group(P<0.001).Conclusion:For patients with cerebrospinal fluid leakage after failed epidural puncture,subarachnoid block as a remedial anesthesia method is more effective than general anesthesia.It has a better postoperative analgesic effect,a lower risk of headache,a faster recovery speed,and a lower incidence of complications.
