摘要
目的探究加减益气祛邪苍耳子汤联合穴位埋线治疗肺气虚寒型季节性变应性鼻炎(SAR)患者疗效观察。方法回顾性分析2022年6月—2023年10月收治的肺气虚寒型季节性变应性鼻炎(SAR)患者的临床资料,随机从接受益气祛邪苍耳子汤治疗的临床资料中抽取42例纳入中药组,从接受穴位埋线治疗的临床资料中抽取42例纳入埋线组,从接受益气祛邪苍耳子汤联合穴位埋线治疗的临床资料抽取42例纳入联合组。比较治疗8周后3组患者的临床疗效差异;比较治疗前及治疗8周后3组患者鼻部症状体征评分(TNSS)、鼻炎伴随症状评分(TNNSS)差异;比较治疗前及治疗4、8周后3组患者生活质量[鼻结膜炎生活质量调查问卷(RQLQ)]差异;随访60 d后,比较3组患者疾病复发情况。结果治疗8周后,联合组临床疗效总有效率显著高于中药组和埋线组(P<0.05),中药组和埋线组之间差异无统计学意义(P>0.05);治疗8周后,3组患者TNSS、TNNSS评分均显著降低(P<0.05),且联合组显著低于中药组和埋线组(P<0.05);治疗4、8周后,3组患者RQLQ评分均较治疗前降低,且联合组显著低于同期中药组和埋线组(P<0.05);随访60 d后,联合组复发率显著低于中药组及埋线组(P<0.05)。结论加减益气祛邪苍耳子汤联合穴位埋线治疗能有效减轻肺气虚寒型SAR患者的相关症状,改善鼻腔生理功能,对患者生活质量具有明显的正面影响,且具有较高的有效性及安全性,复发率较低,为进一步完善SAR的治疗提供参考。
Objective To explore the efficacy of modified Yiqi Quxie Cangerzi Decoction(益气祛邪苍耳子汤)combined with acupoint catgut embedding in the treatment of patients with seasonal allergic rhinitis(SAR)of lung-Qi deficiency cold type.Methods The clinical data of patients with SAR of lung-Qi deficiency cold type admitted to the hospital from June 2022 to October 2023 were retrospectively analyzed.42 cases were randomly selected from the clinical data of patients with Yiqi Quxie Cangerzi Decoction and included in traditional Chinese medicine group,and 42 cases were randomly enrolled from the clinical data of patients with acupoint catgut embedding treatment and assigned as catgut embedding group,and 42 cases who received Yiqi Quxie Cangerzi Decoction combined with acupoint catgut embedding were classified as combined group.The clinical efficacy after 8 weeks of treatment,total nasal symptom scores(TNSS)and total non-nasal symptom scores,(TNNSS)before treatment and after 8 weeks of treatment,quality of life[rhinoconjunctivitis quality of life questionnaire(RQLQ)]before treatment and after 4 weeks and 8 weeks of treatment,disease recurrence after 60 days of follow-up were compared among the three groups.Results After 8 weeks of treatment,the total effective rate of clinical efficacy in combined group was significantly higher than that in traditional Chinese medicine group and catgut embedding group(P<0.05),but there was no significant difference between traditional Chinese medicine group and catgut embedding group(P>0.05).The TNSS scores and TNNSS scores in the three groups were significantly decreased after 8 weeks of treatment(P<0.05),and the scores were significantly lower in combined group than those in traditional Chinese medicine group and catgut embedding group(P<0.05).After 4 and 8 weeks of treatment,the RQLQ score in the three groups was reduced compared with that before treatment,and the score in combined group was significantly lower compared to traditional Chinese medicine group and catgut embedding group during the same period(P<0.05).After 60 days of follow-up,the recurrence rate in combined group was significantly lower than that in traditional Chinese medicine group and catgut embedding group(P<0.05).Conclusion Modified Yiqi Quxie Cangerzi Decoction combined with acupoint catgut embedding therapy can effectively alleviate the related symptoms and improve the nasal physiological function in patients with SAR of lung-Qi deficiency cold type,and it has a significant positive effect on quality of life of patients.It has high effectiveness and safety and low recurrence rate,and it provides a reference for further improving the treatment of SAR.
