摘要
慢性阻塞性肺疾病(COPD)是一种常见的慢性呼吸系统疾病,以慢性咳嗽、咳痰、气短和呼吸困难为主要临床特征。目前西药治疗有一定的疗效,但存在不良反应、停药复发等弊端。中药治疗可以很大程度上避免这类弊端。痰热清具有清热、解毒、化痰的功效,是中医药治疗慢阻肺的代表药物之一。临床研究显示,痰热清不仅能有效地改善慢阻肺患者的临床症状和肺功能指标,还能降低患者炎症因子的水平、提升患者生活质量,且不良反应较小。相关基础研究表明,痰热清治疗慢阻肺涉及多个方面的作用机制,主要包括抗炎、抗氧化和免疫调节等,这些作用机制的发现,为痰热清在慢阻肺的治疗中提供了科学依据。文章系统地梳理了近10年痰热清在慢阻肺治疗中的研究进展,包括临床指南、共识、临床研究与基础研究,文章指出了相关研究的不足并给出了未来研究方向的相关建议,为临床实践和未来的研究提供了理论依据。
Chronic obstructive pulmonary disease(COPD)is a common chronic respiratory system disease,characterized primarily by chronic cough,sputum production,shortness of breath,and difficulty breathing.Current Western medicine treatments have certain therapeutic effects,but they come with drawbacks such as adverse reactions and relapses upon discontinuation.Traditional Chinese medicine treatments can largely avoid these shortcomings.Tanreqing(痰热清)with its effects of clearing heat,detoxifying,and resolving phlegm,is one of the representative drugs in traditional Chinese medicine for treating COPD.Clinical studies have shown that Tanreqing can not only effectively improve the clinical symptoms and pulmonary function indicators of COPD patients but also reduce the levels of inflammatory factors and enhance the quality of life for patients,with mild side effects.Relevant basic research indicates that the treatment of COPD with Tanreqing involves multiple mechanisms,including anti-inflammatory,antioxidant,and immune-regulating effects,which provide a scientific basis for its use in the treatment of COPD.This article systematically reviews the research progress of Tanreqing in the treatment of COPD over the past decade,including clinical guidelines,consensus,clinical studies,and basic research.The article points out the deficiencies in related research and provides suggestions for future research directions,offering a theoretical basis for clinical practice and future studies.
