摘要
目的探讨胸痛确诊主动脉夹层患者临床及计算机断层扫描(CT)平扫特征。方法选择2022年10月至2023年10月阜外华中心血管病医院收治的120例胸痛患者进行研究,根据主动脉夹层情况分为主动脉夹层组(n=54)与非主动脉夹层组(n=66)。分析两组临床特征、胸部CT平扫结果及实验室指标水平。结果主动脉夹层组疼痛呈刀割样/撕裂样比例、收缩压高于非主动脉夹层组,管腔内线样高密度、管腔内弧形高密度、钙化斑块内移比例高于非主动脉夹层组,D-二聚体、载脂蛋白A-Ⅰ水平高于非主动脉夹层组,高密度脂蛋白胆固醇水平低于非主动脉夹层组,差异有统计学意义(P<0.05)。结论疼痛呈刀割样/撕裂样、收缩压升高、管腔内线样高密度、钙化斑块内移、D-D升高为胸痛确诊主动脉夹层患者的临床及CT平扫特征,有助于临床早期快速识别主动脉夹层患者。
Objective To analyze the clinical and computed tomography(CT)plain scan characteristics of patients with chest pain diagnosed with aortic dissection.Methods A total of 120 patients with chest pain admitted to Fuweihua Central Vascular Hospital from October 2022 to October 2023 were selected for the study,and divided into aortic dissection group(n=54)and non-aortic dissection group(n=66)according to the conditions of aortic dissection.The clinical features,chest CT plain scan results and laboratory indexes of the two groups were analyzed.Results The pain of aortic dissection group was higher than that of non-aortic dissection group in terms of knife/tear ratio,the systolic blood pressure,intracavicular linear density,intracavicular arc-shaped density and calcified plaque inward ratio,D-dimer and apolipoprotein A-I levels were higher than those of non-aortic dissection group,and the HDL cholesterol levels were lower than those of non-aortic dissection group.The differences were significant(P<0.05).Conclusions The clinical and CT features of patients with chest pain diagnosed with aortic dissection are knife/tear pain,elevated systolic blood pressure,high intracavity density,calcified plaques,and elevated D-D,which are helpful for rapid identification of patients with aortic dissection in early clinical stage.
