摘要
目的分析血清胱抑素C(CysC)、尿素氮(BUN)及β2-微球蛋白(β2-MG)检测对老年原发性高血压患者早期肾损伤的诊断价值。方法队列研究。采用便利随机抽样法抽取2022年1月至2024年6月郑州颐和医院收治的36例老年原发性高血压患者为研究组,并以1∶1配比抽取36例健康体检者为对照组。检测纳入患者血清CysC、BUN、β2-MG水平。比较研究组与对照组、研究组不同病程患者、研究组中发生与未发生早期肾损伤患者血清CysC、BUN、β2-MG水平。采用Pearson分析血清CysC、BUN、β2-MG水平与老年原发性高血压病程和发生早期肾损伤的相关性。结果研究组血清CysC、BUN、β2-MG水平高于对照组(P<0.05)。研究组老年中原发性高血压病程长期患者血清CysC、BUN、β2-MG水平高于短期患者和中期患者(P<0.05)。研究组中发生早期肾损伤者血清CysC、BUN、β2-MG水平高于未发生早期肾损伤者(P<0.05)。Pearson分析显示,CysC(r=0.74,r=0.95)、BUN(r=0.51,r=0.82)、β2-MG(r=0.70,r=0.97)和老年原发性高血压患者病程、发生早期肾损伤呈正相关(P<0.05)。结论老年原发性高血压患者血清CysC、BUN、β2-MG水平较高,并和原发性高血压病程、发生早期肾损伤密切相关。
Objective To analyze the significance of serum cystatin C(CysC),urea nitrogen(BUN)andβ2-microglobulin(β2-MG)testings in the diagnosis of early kidney injury in elderly patients with essential hypertension.Methods A total of 36 elderly patients with essential hypertension treated in Zhengzhou Yihe Hospital from January 2022 to June 2024 were selected as the study group by convenience random sampling method;36 healthy individuals who underwent physical examination were selected as the control group at a ratio of 1∶1.Serum levels of CysC,BUN andβ2-MG in all selected patients were measured.The serum levels of CysC,BUN andβ2-MG were compared between the study group and the control group,among patients with different courses of essential hypertension in the study group,and between patients with and without early kidney injury in the study group.And the correlations of the levels of serum CysC,BUN andβ2-MG with the disease course of essential hypertension and the occurrence of early renal injury in the elderly patients with essential hypertension were analyzed by Pearson analysis.Results Serum levels of CysC,BUN,andβ2-MG in the study group were higher than those in the control group(P<0.05).In the study group,serum levels of CysC,BUN,andβ2-MG in patients with longer course of essential hypertension were higher compared with patients short-and intermediate-course of essential hypertension(P<0.05).Serum levels of CysC,BUN,andβ2-MG in patients with early kidney injury were higher than those in patient without early kidney injury in the study group(P<0.05).Pearson analysis showed the levels of CysC(r=0.74,r=0.95),BUN(r=0.51,r=0.82)andβ2-MG(r=0.70,r=0.97)had significant correlations with the duration of the disease and the occurrence of early kidney injury in patients with essential hypertension(P<0.05).Conclusions Serum CysC,BUN,andβ2-MG have higher expressions in elderly patients with essential hypertension,and they are closely related to the duration of essential hypertension and the occurrence of early kidney injury.
