摘要
目的分析托吡酯联合重复经颅磁刺激治疗癫痫的效果及对氧化-抗氧化系统的影响。方法队列研究。采取方便抽样法抽取2021年12月至2023年12月郑州大学第三附属医院收治的50例癫痫患者。根据随机数字表法分为对照组与研究组,每组25例。对照组采用托吡酯治疗,研究组采用托吡酯联合重复经颅磁刺激治疗。比较两组临床疗效、氧化-抗氧化系统[胶质酸性纤维蛋白(GFAP)、丙二醛(MDA)、神经元特异性烯醇化酶(NSE)、神经特异性蛋白S-100β(S-100β)、谷胱甘肽过氧化物酶(GSH-Px)、总抗氧化能力(TAC)、超氧化物歧化酶(SOD)]、神经递质[谷氨酸(Glu)、5-羟色胺(5-HT)、多巴胺(DA)]及脑电图(棘波指数、痫样放电数、累及导联数)。结果研究组治疗总有效率(92.00%,23/25)高于对照组(68.00%,17/25),P<0.05。治疗后,研究组GFAP、MDA、NSE、S-100β蛋白水平低于对照组,GSH-Px、TAC、SOD水平高于对照组(P<0.05)。治疗后,研究组Glu水平低于对照组,5-HT及DA水平高于对照组(P<0.05)。治疗后,研究组棘波指数、痫样放电数与累及导联数均低于对照组(P均<0.05)。结论托吡酯联合重复经颅磁刺激对癫痫的治疗效果显著,有助于改善氧化-抗氧化系统、神经递质,并促进脑电图指标恢复。
Objective To analyze the effect of topiramate combined with repetitive transcranial magnetic stimulation in the treatment of epilepsy,and to investigate its influence on the oxidation-antioxidant system.Methods A total of 50 patients with epilepsy admitted to the Third Affiliated Hospital of Zhengzhou University from December 2021 to December 2023 were selected by convenient sampling method,and they were divided into the control group and the study group by random number table method,with 25 cases in each group.The control group was treated with topiramate,and the study group was treated by repetitive transcranial magnetic stimulation combined with topiramate.The clinical efficacy,oxidation-antioxidant system evaluated by glial fibrillary acidic protein(GFAP),malondialdehyde(MDA),neuron specific enolase(NSE),neurospecific protein S-100β(S-100β),glutathione peroxidase(GSH-Px),total antioxidant capacity(TAC)and superoxide dismutase(SOD),neurotransmitters including glutamic acid(Glu),5-hydroxytryptamin(5-HT)and dopamine(DA),and electroencephalogram evaluated by spike-wave index,the number of epileptic discharges and the number of involved leads were compared between the two groups.Results The total effective rate in the study group(92.00%,23/25)was higher than that in the control group(68.00%,17/25),P<0.05.After treatment,the study group had lower levels of GFAP,MDA,NSE and S-100βthan the control group,moreover,the study group had higher levels of GSH-Px,TAC and SOD(P<0.05).After treatment,the study group had lower Glu level,and had higher levels of 5-HT and DA than the control group(P<0.05).After treatment,the spike-wave index,the number of epileptic discharges and the number of involved leads of the study group were significantly lower than those of the control group(all P<0.05).Conclusions In the treatment of epilepsy,the combination of topiramate and repetitive transcranial magnetic stimulation has remarkable therapeutic effects;it is conductive to improving oxidation-antioxidant system,neurotransmitters,and the recovery of electroencephalogram indicators.
