摘要
目的探讨平均血小板体积与淋巴细胞比值(MPVLR)对肝硬化(LC)患者并发上消化道出血的预测价值。方法回顾性抽取2020年3月至2023年3月西安医学院附属宝鸡医院收治的120例LC患者,按是否出现上消化道出血分为出血组(48例)和非出血组(72例)。检测两组患者平均血小板体积(MPV)、淋巴细胞计数(LYC)并计算MPVLR;采用受试者工作特性曲线评估MPVLR对患者上消化道出血的预测价值。结果非出血组患者MPV、LYC、MPVLR均低于出血组(P均<0.05)。MPV、LYC、MPVLR预测LC患者并发上消化道出血的AUC分别为0.814(95%CI:0.732~0.875)、0.778(95%CI:0.720~0.851)、0.910(95%CI:0.847~0.944),截点值分别为12.84 fL、79.46%、0.31,特异度分别为61.22%、56.10%、89.20%,灵敏度分别为91.45%、91.45%、86.57%。结论MPVLR上升提示LC患者可能并发上消化道出血,临床可将其作为预测指标。
Objective To investigate the predictive value of mean platelet volume to lymphocyte ratio(MPVLR)for upper gastrointestinal bleeding in patients with liver cirrhosis(LC).Methods A total of 120 LC patients admitted to Baoji Hospital Affiliated to Xi’an Medical University from March 2020 to March 2023 were retrospectively selected,and they were divided into a bleeding group(48 cases)and a non-bleeding group(72 cases)based on the occurrence of upper gastrointestinal bleeding.The mean platelet volume(MPV),lymphocyte count(LYC)of the two groups were detected,and MPVLR was calculated.The predictive value of MPVLR for upper gastrointestinal bleeding in patients was evaluated using receiver operating characteristic curve.Results MPV,LYC,and MPVLR in the non-bleeding group were significantly lower than those in the bleeding group(all P<0.05).The area under the curve(AUC)of MPV,LYC,and MPVLR in predicting upper gastrointestinal bleeding in LC patients were 0.814(95%CI:0.732-0.875),0.778(95%CI:0.720-0.851),and 0.910(95%CI:0.847-0.944),respectively;the cutoff values were 12.84 fL,79.46%,and 0.31,respectively.The specificities were 61.22%,56.10%,and 89.20%,respectively,while the sensitivities were 91.45%,91.45%,and 86.57%,respectively.Conclusions An elevated MPVLR indicates a potential risk of upper gastrointestinal bleeding in LC patients,and it can be used as a predictive indicator in clinical practice.
