摘要
目的调查膀胱癌尿流改道腹壁造口患者社会疏离的潜在类别并分析影响因素。方法采用便利抽样法选取2023年11月至2024年4月新疆某医院膀胱癌尿流改道腹壁造口术后出院患者150例为研究对象,使用一般资料调查表、一般疏离感量表、一般自我效能感量表、社会功能缺陷筛选表进行评估,采用潜在剖面分析研究其社会疏离的类别及特征,并通过单因素分析和多元Logisitc回归分析不同类别的影响因素。结果膀胱癌尿流改道腹壁造口患者社会疏离可分为低水平组(34.10%)、中水平组(32.57%)、高水平组(33.33%)3个类别。Logistic回归分析显示,文化程度、家庭人均月收入、居住方式、自我效能感、社会参与度均是术后社会疏离程度的影响因素(P<0.05)。结论膀胱癌尿流改道腹壁造口术后患者社会疏离存在3个类别,护理人员应结合不同潜在类别及其主要影响因素制订针对性多维度的干预措施,降低患者社会疏离。
Objective To investigate the latent profile of social isolation on bladder cancer patients undergoing urinary diversion with abdominal stoma and analyze the influencing factors.Methods A convenience sampling method was used to selected a total of 150 postoperative patients discharged from a Hospital in Xinjiang from November 2023 to January 2024.The patients were assessed by used the general information questionnaire,General Isolation Scale,General Self-Efficacy Scale,and Social Dysfunction Screening Scale.The latent profile analysis was used to identified the categories and characteristics of social isolation,while univariate analysis and multivariate Logistic regression were employed to examined the influencing factors of different latent profiles.Results The 3 latent profiles of social isolation were identified among bladder cancer patients with urinary diversion through abdominal stoma:low-level(34.10%),moderate-level(32.57%),and high-level(33.33%)groups.The Logistic regression analysis showed that education level,monthly postoperative income,living arrangement,self-efficacy,and social participation were significant influencing factors of postoperative social isolation(P<0.05).Conclusion Bladder cancer patients undergoing urinary diversion with abdominal stoma experience 3 distinct categories of social isolation.Nursing staff should develop targeted,multidimensional interventions based on different latent profiles and their major influencing factors to reduce social isolation in these patients.
