摘要
目的探讨热塑体膜固定对乳腺癌保乳术后放疗摆位准确性及其影响因素。方法选取乳腺癌保乳术后患者67例,分为观察组(n=36)和对照组(n=31)。观察组采用热塑体膜固定,对照组采用乳腺托架固定。通过多层螺旋CT比较两组摆位误差,比较两组生活质量和不良反应情况,并分析其影响因素。结果观察组头脚轴向的平移和旋转摆位误差均低于对照组(P<0.05),体质指数是头脚轴向旋转摆位误差的影响因素(β值为-0.280,P<0.05)。治疗后,两组SF-36量表各维度评分均较治疗前升高(P<0.05),且观察组躯体疼痛、生理功能和总体健康评分高于对照组(P<0.05)。两组放射性皮炎发生率差异无显著性(P>0.05)。结论热塑体膜固定可减少乳腺癌保乳术后放疗的摆位误差,头脚轴向旋转摆位误差可能受患者体质指数影响。
Aim To investigate the effect of thermoplastic film fixation on setup accuracy and influencing factors in radiotherapy after breast-conserving surgery.Methods A total of 67 patients after breast-conserving surgery were divided into an observation group(n=36)and a control group(n=31).The observation group received thermoplastic film fixation,while the control group received breast bracket fixation.The setup errors between the two groups using multi-slice spiral CT were compared,the differences in quality of life and adverse reactions were assessed,and the influencing factors were analyzed.Results The translational and rotational setup errors in the craniocaudal direction were lower in the observation group than those in the control group(P<0.05).Body mass index was an influencing factor for rotational setup errors in the craniocaudal direction(β=-0.280,P<0.05).After treatment,SF-36 scores improved in both groups(P<0.05),with higher scores in bodily pain,physical function,and general health in the observation group(P<0.05).No significant difference was found in the incidence of radiation dermatitis between the two groups(P>0.05).Conclusion Thermoplastic film fixation can reduce setup errors in radiotherapy after breast-conserving surgery,and rotational setup errors in the craniocaudal direction may be influenced by body mass index.
