摘要
目的探讨胃液pH值、表皮生长因子(EGF)、前列腺素E2(PGE2)对胃溃疡出血患者内镜治疗效果的影响及其与血清胃肠激素的相关性。方法选取胃溃疡出血患者84例,根据内镜疗效分为有效组(58例)和无效组(26例)。检测两组胃液pH值、EGF、PGE2及血清胃肠激素[胃泌素(GAS)、胃动素(MTL)、血管活性肠肽(VIP)]水平;分析胃液pH值、EGF、PGE2与胃肠激素的相关性;采用多因素Logistic回归分析内镜疗效的影响因素;通过ROC曲线评估胃液各指标对内镜疗效的预测价值。结果有效组GAS、MTL、VIP低于无效组,胃液pH值、EGF、PGE2高于无效组(P<0.05)。胃液pH值、EGF、PGE2与GAS、MTL、VIP呈负相关(P<0.05)。胃液pH值、EGF、PGE2是内镜疗效的独立影响因素(P<0.05)。ROC分析显示三者联合预测内镜疗效的AUC高于单独指标预测值。结论胃液pH值、EGF、PGE2水平与胃溃疡出血患者内镜疗效及血清胃肠激素变化相关,胃液各指标联合检测预测内镜疗效的价值较高。
Aim To investigate the correlation of gastric pH,epidermal growth factor(EGF),prostaglandin E2(PGE2)levels with endoscopic efficacy and gastrointestinal hormones in gastric ulcer bleeding patients.Methods Totally 84 gastric ulcer bleeding patients were divided into an effective group(58 cases)and an ineffective group(26 cases)according to endoscopic efficacy.The levels of gastric pH,EGF,PGE2 and gastrointestinal hormones[serum gastrin(GAS),motilin(MTL),and vasoactive intestinal peptide(VIP)]were detected.The correlations were analyzed by Pearson method.Multivariate Logistic regression was used to analyze the influencing factors for endoscopic therapeutic efficacy.ROC curve was used to evaluate the predictive value for endoscopic therapeutic efficacy.Results The effective group had lower GAS,MTL,VIP levels but higher gastric pH,EGF,PGE2 than those in the ineffective group(P<0.05).Gastric pH,EGF and PGE2 levels were negatively correlated with GAS,MTL and VIP levels(P<0.05).Gastric pH,EGF and PGE2 levels were independent influencing factors for endoscopic therapeutic efficacy(P<0.05).ROC analysis showed that the combined prediction using the three indicators had a higher AUC than any single index.Conclusion Gastric pH,EGF and PGE2 levels are related to endoscopic efficacy and gastrointestinal hormones in gastric ulcer bleeding patients,and their combination has higher predictive value.
