摘要
目的探讨光学相干断层扫描血管成像(OCTA)联合眼底荧光血管造影(FFA)检测对糖尿病性视网膜病变(DR)严重程度的诊断价值。方法回顾性纳入本院就诊的糖尿病患者831例。所有患者均进行OCTA、FFA及眼底检查。以眼底检查为金标准,分析OCTA、FFA以及OCTA联合FFA检测不同DR严重程度的灵敏度、特异度、准确率,并采用Kappa值进行一致性分析。比较不同DR严重程度患者视网膜神经纤维层(RNFL)厚度和黄斑中心凹下脉络膜厚度(SFCT)。结果OCTA与FFA联合检测不同DR严重程度的灵敏度及特异度均高于单一检测值,且与金标准具有高度一致性。增殖性DR患者RNFL、SFCT低于非增殖性DR和无DR患者(P<0.05),非增殖性DR患者RNFL、SFCT低于无DR者(P<0.05)。结论OCTA联合FFA检测对DR严重程度具有一定的诊断价值。
Aim To evaluate the diagnostic value of optical coherence tomography angiography(OCTA)and combined with fundus fluorescein angiography(FFA)in determining the severity of diabetic retinopathy(DR).Methods A total of 831 patients with diabetes retrospectively enrolled from our hospital were included.All patients were underwent OCTA,FFA,and fundus examination.Using fundus examination as the gold standard,the sensitivity,specificity,and accuracy of OCTA,FFA,and OCTA combined with FFA in detecting the severity of different DR lesions were analyzed,and Kappa values were used for consistency analysis.The retinal nerve fiber layer(RNFL)thickness and subfoveal choroidal thickness(SFCT)in patients with different degrees of DR lesions were compared.Results The sensitivity and specificity of OCTA combined with FFA in detecting different degrees of DR were higher than single detection values,with high consistence with the gold standard.The RNFL and SFCT of proliferative DR patients were lower than those of non-proliferative DR and non-DR patients(P<0.05),and the RNFL and SFCT of non-proliferative DR patients were lower than those of non-DR patients(P<0.05).Conclusion The detection of OCTA combined with FFA has certain diagnostic value for the severity of DR.
