摘要
目的探讨孕中期多普勒超声检测胎儿大脑中动脉(MCA)、脐动脉(UA)血流参数对子痫前期(PE)发病的预测价值。方法选择PE患者130例(PE组)和非PE孕妇130例(非PE组)。采用彩色多普勒超声检测两组MCA和UA的搏动指数(PI)及收缩期峰值流速与舒张末期流速之比(S/D)。采用多因素Logistic回归分析PE发病的危险因素,采用ROC曲线评估各指标对PE的预测价值。结果PE组舒张压、收缩压、既往PE史占比、PE家族史占比高于非PE组(P<0.05)。PE组MCA的PI及S/D低于非PE组,UA的PI及S/D高于非PE组(P<0.05)。多因素Logistic回归分析显示UA的PI和S/D是PE的危险因素,MCA的PI和S/D是PE的保护因素(P<0.05)。ROC曲线分析显示各指标联合预测的AUC为0.948,优于单独指标预测值(P<0.05)。结论孕中期多普勒超声检测MCA、UA血流参数对PE发病具有较高预测价值。
Aim To investigate the predictive value of second trimester ultrasound Doppler detection of middle cerebral artery(MCA)and umbilical artery(UA)blood flow parameters for preeclampsia(PE).Methods Totally 130 PE patients(PE group)and 130 non-PE pregnant women(non-PE group)were selected.Color Doppler ultrasound was used to detect MCA and UA pulsatility index(PI)and systolic to diastolic ratio(S/D).Multivariate Logistic regression was used to analyze risk factors for PE,and ROC curve was used to evaluate predictive value.Results Diastolic pressure,systolic pressure,proportion of prior PE history and family history of PE in the PE group were higher than those in the non-PE group(P<0.05).PI and S/D of MCA in the PE group were lower than those in the non-PE group,while PI and S/D of UA were higher(P<0.05).Multivariate Logistic regression showed that PI and S/D of UA were risk factors,while PI and S/D of MCA were protective factors for PE(P<0.05).ROC analysis showed the combined prediction AUC was 0.948,better than that of single indicator(P<0.05).Conclusion Second trimester ultrasound Doppler detection of MCA and UA blood flow parameters has high predictive value for PE.
