摘要
卒中后癫痫作为中风与痫病并病的复合疾病,具有症状多样、病机复杂的特点,目前各学派或医家辨治该病各有侧重,尚未形成规范化诊疗标准。“以证统病”理论是将多种同质性疾病归纳在证之下,研究病证的发生、发展、治疗及转归。本文将“以证统病”理论作为指导思想,通过四诊合参归纳卒中后癫痫的证素得出:“风”为触发此病的先决条件,“火”是此病的诱发因素,“痰”“瘀”乃此病治疗的重点和难点,“虚”是此病缠绵难愈的症结所在。因而治疗该病时,应在辨治证素的基础上,兼顾疾病特殊性遣方用药。这可弥补卒中后癫痫证治纷杂、难以统一的不足,优化卒中后癫痫中医特色诊疗模式。
Post-stroke epilepsy,as a combined disease of stroke and epilepsy,is characterized by diverse symptoms and complex pathogenesis.Currently,various schools of thought or medical practitioners have their own focuses in diagnosing and treating this disease,and a standardized diagnosis and treatment standard has not yet been formed.The theory of"syndrome dominating disease"categorizes various homogeneous diseases under syndrome,and studies the occurrence,development,treatment and prognosis of diseases and syndromes.This article takes the theory of"syndrome dominating disease"as the guiding ideology,and summarizes the syndrome elements of post-stroke epilepsy through the combination of four diagnostic methods.It is concluded that"wind"is the prerequisite for triggering this disease,"fire"is the triggering factor of this disease,"phlegm"and"stasis"are the key and difficult points in the treatment of this disease,and"deficiency"is the lingering and difficult crux of this disease.Therefore,when treating this disease,it is necessary to take into account the specific characteristics of the disease and prescribe medication based on the differentiation and treatment of syndrome elements.This can make up for the deficiency of the complex and difficult-to-unify syndromes and treatments of post-stroke epilepsy,and optimize the characteristic diagnosis and treatment model of traditional Chinese medicine for post-stroke epilepsy.
