摘要
目的 探究小青龙汤合小柴胡汤加减治疗慢性阻塞性肺疾病(COPD)稳定期患者的临床疗效。方法 选取76例COPD稳定期患者为研究对象,通过随机数表将患者分为观察组和对照组,每组38例。对照组患者接受常规西医治疗,观察组在对照组的基础上联合应用小青龙汤合小柴胡汤加减治疗,连续治疗12周。比较两组患者临床疗效,干预前后中医证候积分、气道功能、免疫功能及生活质量。结果 观察组的治疗总有效率高于对照组(P<0.05)。干预后,观察组咳嗽、咳痰、喘息、乏力和自汗证候积分低于对照组,最大深呼吸第1秒呼出气量容积(FEV_(1))、FEV_(1)和用力肺活量(FVC)的比值、剩50%肺活量时的最大呼气流速和中段呼气流速(MEF_(25~75))水平高于对照组(P<0.05);干预后,观察组的CD3^(+)、CD4^(+)、IgG、IgA和IgM水平高于对照组,CD8^(+)水平低于对照组,慢性阻塞性肺病评估测试(CAT)评分低于对照组,改良Barthel指数(MBI)评分高于对照组(P<0.05)。结论 小青龙汤合小柴胡汤加减治疗可有效改善COPD稳定期患者的临床症状,促进气道功能恢复,增强免疫功能,进而提高患者生活质量。
Objective To investigate the clinical effect of modified Xiaoqinglong Decoction and Xiaochaihu Decoction in the treatment of patients with stable chronic obstructive pulmonary disease(COPD).Methods A total of 76 patients with stable COPD were selected as the research objects.According to random number table method,they were divided into observation group and control group,with 38 patients in each group.Patients in the control group were given conventional western medicine treatment.On this basis,patients in the observation group were treated with modified Xiaoqinglong Decoction and Xiaochaihu Decoction.All patients underwent 12 weeks of continuous treatment.Clinical efficacy and changes in traditional chinese medicine syndrome scores,airway function,immune function and quality of life before and after treatment were compared between the two groups.Results The total effective rate of treatment in the observation group was higher than that in the control group(P<0.05).After treatment,the scores for cough,expectoration,wheezing,fatigue and spontaneous sweating of the observation group were lower than those of the control group.FEV 1,FEV 1/FVC,maximum expiratory flow rate at 50%vital capacity and maximum expiratory flow between 25%and 75%of FVC were higher than those in the control group(P<0.05).After treatment,the levels of CD3^(+),CD4^(+),IgG,IgA and IgM in the observation group were higher than those in control group.CD8^(+)level and COPD assessment test(CAT)scores were lower than those in the control group,and modified Barthel index(MBI)score was higher than that in the control group(P<0.05).Conclusion The treatment with modified Xiaoqinglong Decoction and Xiaochaihu Decoction for patients with stable COPD can effectively improve clinical symptoms,promote the recovery of airway function and enhance immune function,thus improving the quality of life.
