摘要
目的:探讨不同经线宽度结膜植片在自体角膜缘干细胞移植治疗翼状胬肉中的应用效果。方法:选取2021年2月—2023年2月丰县人民医院收治的翼状胬肉患者100例作为研究对象,随机分为对照组和观察组,每组50例。对照组从球结膜区取经线宽度为3 mm的结膜植片,观察组从球结膜区取经线宽度为5 mm的结膜植片。比较两组治疗效果。结果:观察组治疗总有效率高于对照组(P=0.025)。术后2d、3d、5d,两组角膜刺激症状评分逐渐降低(P<0.05),且观察组低于对照组(P<0.001)。术后6个月、1年、2年,观察组复发率低于对照组(P<0.05)。术后,两组裸眼视力、5min试纸润湿长度、泪膜破裂时间增加(P<0.05),且观察组增加幅度大于对照组(P<0.001);两组角膜屈光度降低(P<0.05),且观察组低于对照组(P<0.001)。结论:相较于3 mm经线宽度的结膜植片,5 mm经线宽度的结膜植片在自体角膜缘干细胞移植治疗翼状胬肉中的应用效果更佳,可减轻角膜刺激症状,改善临床指标,降低复发率。
Objective:To explore the application effect of autologous corneal limbal stem cell transplantation with conjunctival grafts of different longitude widths in treatment of pterygium.Methods:A total of 100 patients with pterygium admitted to Feng Xian People's Hospital from February 2021 to February 2023 were selected as the study subjects.They were randomly divided into control group and observation group,with 50 cases in each group.In the control group,conjunctival grafts with a longitude width of 3 mm were harvested from the bulbar conjunctival area.In the observation group,conjunctival grafts with a longitude width of 5 mm were harvested from the bulbar conjunctival area.The treatment effects were compared between the two groups.Results:The overall response rate in the observation group was higher than that in the control group(P=0.025).On 2 d,3 d and 5 d after surgery,the corneal irritation symptom scores in the two groups decreased gradually(P<0.05),and the scores in the observation group were lower than those in the control group(P<0.001).At 6 months,1 year and 2 years after surgery,the recurrence rate in the observation group was lower than that in the control group(P<0.05).After surgery,the naked vision,5-min strip wetting length and tear film breakup time increased in the two groups(P<0.05),and the increase in the observation group was greater than that in the control group(P<0.001).The corneal refraction decreased in the two groups(P<0.05),and this parameter in the observation group was lower than that in the control group(P<0.001).Conclusion:Compared with the conjunctival grafts with a longitude width of 3 mm,the conjunctival grafts with a longitude width of 5 mm has a better effect in the treatment of pterygium by autologous corneal limbal stem cell transplantation,and can relieve the corneal irritation symptoms,improve the clinical indicators,and reduce the recurrence rate.
