摘要
目的:总结老年住院病人跌倒风险筛查与评估的最佳证据,为我国临床实践提供参考依据。方法:采用PIPOST构建循证问题,计算机检索有关老年住院病人跌倒风险筛查与评估的证据资源,包括指南、证据总结、最佳实践信息册、专家共识、系统评价、随机对照研究。由4名完成循证护理培训课程的研究者分别进行文献筛选、文献质量评价、证据级别划分、证据提取及汇总。结果:最终纳入11篇文献,包括临床决策1篇,指南3篇,系统评价1篇,专家共识4篇,证据总结2篇,从老年住院病人跌倒风险初步筛查、跌倒风险多因素评估2个主题总结了17条最佳证据。结论:汇总出的老年住院病人跌倒风险筛查与评估的最佳证据可准确识别跌倒高危因素,为制订适合老年住院病人预防跌倒的个性化措施提供循证依据。
Objective:To summarize the best evidence for the assessment and management of falls in hospitalized elderly patients,and to provide reference for clinical practice in China.Methods:With PIPOST model,the evidencebased questions were identified;computerbased retrieval of evidence resources for falls assessment and management including guidelines,summary of evidence,best practice information book,expert consensus,systematic review,literature review,and randomized controlled trials in hospitalized elderly patients was performed.Literature screening,literature quality evaluation,classification of evidence grade and recommendation grade,evidence extraction and summary were conducted by 4 researchers who completed the evidencebased nursing training.Results:Eleven articles were included,including 1 clinical decisionmaking,3 guidelines,1 systematic review,4 expert consensus reports,and 2 evidence summary report and 17 pieces of best evidence were summarized from 2 dimensions of preliminary screening of fall risk in elderly hospitalized patients and multi factor assessment of fall risk.Conclusions:The best evidence of fall risk screening and assessment of elderly inpatients can accurately identify the highrisk factors of falls,and provide evidencebased basis for the development of personalized measures for the prevention of falls in elderly inpatients.
