北京某医院普通病房功能物品表面监测情况分析

摘要 目的分析北京地区某三级医院普通病房功能物品携带细菌情况,为制定院内感染预防控制策略提供参考依据。方法对2019年1月至2021年12月北京市某三级医院普通病房20种功能物品表面进行取样,送至检验科,进行细菌培养,计算菌落数,鉴定阳性样本菌株,并与同期普通病房发生的317例院感患者病原菌进行对比。结果总共430次采样,71次检出细菌,总检出率为16.51%,前3位分别为治疗车背阴处(66.67%)、发热患者床单(57.14%)和洗手液按压头(33.33%),以表皮葡萄球菌(26.76%)、人葡萄球菌(15.49%)、阴沟肠杆菌(14.08%)为主。秋季检出率明显高于春季、夏季、冬季(P<0.01),2019年检出率高于2020年和2021年(P<0.001)。同期普通病房院感患者中也检出表皮葡萄球菌(6.62%)、人葡萄球菌(2.21%)、阴沟肠杆菌(5.99%)。结论普通病房功能物品表面携带细菌情况在物品种类、年份、季节和菌株方面存在明显差异,对于易携带细菌的功能物品应加大消毒力度,减少院内感染的发生。 Objective To analyze the bacteria carried by functional items in the general ward of a tertiary hospital in Beijing,so as to provide a reference basis for formulating the prevention and control strategies of nosocomial infection.Methods Study samples were collected from the surface of twenty types of functional items in the general wards of a tertiary hospital in Beijing from January 2019 to December 2021.These samples were sent to the laboratory for bacterial culture.The number of bacterial colonies was calculated,and the strains of positive samples were identified.The rate and related summary statistics were then compared with the pathogens from 317 inpatients with nosocomial infection in general wards during the same period.Results A total of 430 samples were collected,and the bacteria were detected in 71 samples.The total detection rate was 16.51%.The top three functional items carrying the bacteria were the back of the treatment cart(66.67%),the bed sheets of febrile patients(57.14%),and the hand sanitizer pump heads(33.33%).Furthermore,the detection rate of Staphylococcus epidermidis was the highest,accounting for 26.76%,followed by Staphylococcus hominis(15.49%)and Enterobacter cloacae(14.08%).In addition,the detection rate in autumn was significantly higher than that in spring,summer and winter(P<0.01).The detection rate in 2019 was higher than that in 2020 and 2021(P<0.001).Staphylococcus epidermidis(6.62%),Staphylococcus hominis(2.21%)and Enterobacter cloacae(5.99%)were also detected among inpatients in general wards during the same period.The bacteria carried by functional items in the general ward show significant differences in the item types,years,seasons and strains.For functional items that are prone to carry bacteria,it is necessary to increase the intensity of disinfection to reduce the occurrence of nosocomial infection.
