摘要
目的:了解甘肃省养老机构入住老年人对健康照护服务的满意度并探讨其影响因素,为西北地区积极应对人口老龄化提供实证参考。方法:采用多阶段分层随机抽样,对甘肃省7个地州市24个县区50家养老机构的658名入住老年人进行问卷调查,通过主成分因子分析法计算老年人健康照护满意度综合得分,采用χ^(2)检验和无序多分类Logistic回归分析老年人健康照护满意度的影响因素。结果:63.2%的老年人对机构健康照护满意度评价一般,28.7%表示满意,8.1%表示不满意;老年人对机构服务人员素质满意度最高,得分为4.20±0.597分,心理慰藉服务满意度最低,得分为3.57±0.814分;多元Logistic回归分析结果显示:地区、婚姻状况、养老保险参保情况、子女数量、慢性病患病情况及医疗养老需求满足程度显著影响老年人对机构健康照护服务的满意度(P<0.05)。结论:养老机构老年人对健康照护服务的满意度处于中等水平,可从重视老年心理健康、提供情感支持服务、实施个性化分级照护、推进医养结合服务、优化老年社会保障制度等方面入手满足老年人多元化需求,从而提高入住老年人的满意度。
Objective To explore the satisfaction of elderly people with health care in pension institutions in Gansu Province and their influencing factors,and provide empirical references for the northwest region to actively cope with the aging of population.Methods A multi-stage stratified random sampling method was used to conduct a questionnaire survey on 658 elderly people staying in 50 pension institutions in 24 counties and districts of 7 prefectures and cities in Gansu Province.The overall score of health care satisfaction was calculated by principal component factor analysis,and the influencing factors were analyzed by chi-square test and unordered multi-categorical logistic regression analysis.Results 63.2%of the elderly people assessed their satisfaction with institutional health care as general,28.7%were satisfied,and 8.1%were dissatisfied.The highest satisfaction score of the elderly was on the quality of institutional nursing staff,which was 4.20±0.597,while the lowest satisfaction score was on psychological comfort services,which was 3.57±0.814.Multiple logistic regression analysis showed that region,marital status,pension insurance participation,number of children,chronic disease prevalence,and fulfllment of medical and elderly care needs significantly influenced the satisfaction of elderly people with institutional health care services(P<0.05).Conclusion The elderly peoples satisfaction with health care services in pension institutions is at a moderate level.The multifaceted needs of the elderly can be met by valuing the geriatric mental health,providing emotional support services,implementing personalized and graded care,promoting integrated medical and elderly care services,and optimizing the social security system for the elderly.
