摘要
目的了解凉山州高血压患者治疗依从性现状及其影响因素,为开展有针对性的健康教育措施提供科学依据。方法于2023年6月,在凉山州德昌县与盐源县两地,对591名基本公共卫生服务项目在管的原发性高血压患者进行高血压治疗情况现场问卷调查,采用高血压治疗依从性量表评价患者治疗依从性,利用SPSS 18.0软件进行统计分析,单因素分析采用t检验和方差分析,多因素分析采用二元Logistic回归分析,以P<0.05为差异有统计学意义。结果共计回收有效问卷517份,有效率为87.48%,其中男性占42.36%,女性占57.64%,文化程度普遍较低,职业以农民为主;治疗依从性总分为(99.43±25.08)分。单因素分析结果显示,地区、性别、民族、职业、高血压诊断年限、服药种类、服用降压药最高次数对治疗依从性有影响(P均<0.05)。多因素Logistic回归分析结果显示,影响高血压患者治疗依从性因素从高到低依次为服用降压药最高次数(OR=9.231,P=0.008)、优先购药途径为县级及以上综合医院(OR=4.207,P<0.001)、优先购药途径为专科医院(OR=2.648,P<0.05)、合并糖尿病(OR=2.367,P=0.034)、地区(OR=2.17,P=0.003)以及服药种类(OR=2.064,P=0.015)。结论凉山州在管高血压患者治疗依从性需进一步提高,治疗依从性的主要影响因素为服用降压药最高次数、优先购药途径、地区、服用降压药种类以及合并糖尿病。凉山州应充分发挥县级及以上综合医疗机构在基层高血压患者健康管理中的指导作用,加强对经济水平较落后地区、以及尚未发生合并症的高血压患者的健康教育,使其了解按规则服药的重要性,提高其治疗依从性。
Objective To investigate the current status and influencing factors of treatment compliance in patients with hypertension in Liangshan prefecture,and to provide scientific basis for carrying out targeted health education measures.Methods In June 2023,in Dechang County and Yanyuan County of Liangshan Prefecture,an on-site questionnaire survey was conducted on the treatment of hypertension among 591 patients with essential hypertension under the management of basic public health services.The treatment compliance scale was used to evaluate the treatment compliance of patients.SPSS 18.0 software was used for statistical analysis.T-test and ANOVA were used for univariate analysis,and binary Logistic regression analysis was used for multivariate analysis.The difference P<0.05 was considered statistically significant.Results A total of 517 valid questionnaires were collected,and the effective rate was 87.48%.Among them,42.36%were males and 57.64%were females,the education level was generally low,and the occupation was mainly farmers.The total score of treatment compliance was(99.43±25.08).Single factor analysis results showed that the region,sex,nationality,occupation,fixed number of year of the hypertension diagnosis,medication categories and the highest number of times taking blood pressure medication those were affected treatment adherence(all P<0.05).The results of multivariate Logistic regression analysis showed that the factors affecting the treatment compliance of hypertension patients from high to low were the highest frequency of taking antihypertensive drugs(OR=9.231,P<0.001),priority drug purchase way was county level and above general hospitals(OR=4.207,P<0.001),priority drug purchase way was specialized subject hospital(OR=2.648,P=0.008),diabetes mellitus(OR=2.367,P=0.034),the region(OR=2.17,P=0.003)and drug types(OR=2.064,P=0.015).Conclusions The managment on the hypertension treatment adherence in Liangshan Prefecture need to further improve,the main influencing factors of treatment compliance are the highest times of taking blood pressure medication treatment adherence,priority drug purchase way,region,taking a variety of blood pressure medication and diabetes mellitus.Liangshan Prefecture should give full play to the comprehensive medical institutions at the county level and above in the management of primary hypertension patients health guidance,strengthen the economic level is relatively backward area and those have not yet occurred complications of hypertension patients health education,so that to make them understand the importance of taking according to rule,to improve the treatment compliance.
