摘要
目的探讨2型糖尿病合并血栓性疾病患者血清Sestrin2蛋白和微量五元素(Ca、Mg、Fe、Cu和Zn)的表达水平与其他临床指标的关系。方法收集兰州市某三甲医院的2型糖尿病患者(T2DM组),2型糖尿病合并血栓性疾病患者(T2DM+T组)和同期住院的其他非相关疾病患者(Control组)的基本资料,采集研究对象的静脉血,用全自动分析仪测定血糖和凝血指标,用双抗体夹心ELISA法检测血清中Sestrin2水平,用原子吸收法检测血清微量五元素含量。结果三组间在FPG、GSP、APTT、PT、TT、FIB、FDP和D-Dimer上的差异具有统计学意义(P<0.05)。与Control组相比,T2DM组和T2DM+T组Sestrin2、Cu、Ca和Fe水平升高,Zn和Mg的水平降低(P<0.05)。Logistics回归显示,年龄、FIB和Ca是T2DM的危险因素(P<0.05);年龄、FIB和Sestrin2是T2DM+T的危险因素(P<0.05),Mg是其保护因素(P<0.05)。相关性分析中,Sestrin2与Zn负相关(r=-0.182),Cu与GSP、FIB和D-Dimer正相关(r=0.213、0.262和0.171),Zn与FIB和FDP负相关(r=-0.195和-0.198),Ca与FPG、GSP和Zn正相关(r=0.163、0.209和0.389),Mg与Zn和Ca正相关(r=0.310和0.230),Fe与Zn负相关(r=-0.193),均具有统计学意义。结论2型糖尿病和2型糖尿病合并血栓性疾病患者体内Sestrin2水平升高,存在凝血功能异常和微量元素稳态失衡的现象。对2名糖尿病患者进行Sestrin2、微量五元素、凝血指标的检测可提示血栓的发生,有利于2型糖尿病血栓的早期干预。
Objective To explore the relationship between serum Sestrin2 levels,five trace elements(Ca,Mg,Fe,Cu,and Zn),and other clinical indicators in patients with type 2 diabetes mellitus(T2DM)complicated with thrombotic diseases.Methods Basic clinical data were collected from patients with T2DM(T2DM group),patients with T2DM complicated with thrombotic diseases(T2DM+T group),and patients hospitalized for unrelated diseases(Control group)at a tertiary hospital in Lanzhou City.Venous blood samples were analyzed using an automatic analyzer for blood glucose and coagulation indices.Serum Sestrin2 levels were measured by a double-antibody sandwich ELISA method,and the levels of five trace elements were determined by atomic absorption spectrometry.Results Statistically significant differences(P<0.05)were observed among the three groups in fasting plasma glucose(FPG),glycated serum protein(GSP),activated partial thromboplastin time(APTT),prothrombin time(PT),thrombin time(TT),fibrinogen(FIB),fibrin degradation products(FDP),and D-dimer levels.Compared with the Control group,the T2DM and T2DM+T groups had significantly higher levels of Sestrin2,Cu,Ca,and Fe,and lower levels of Zn and Mg(P<0.05).Logistic regression analysis indicated that age,FIB,and Ca were risk factors for T2DM(P<0.05),while age,FIB,and Sestrin2 were risk factors and Mg was a protective factor for T2DM+T(P<0.05).Correlation analysis revealed that Sestrin2 was negatively correlated with Zn(r=-0.182);Cu was positively correlated with GSP,FIB,and D-dimer(r=0.213,0.262,and 0.171,respectively);Zn was negatively correlated with FIB and FDP(r=-0.195 and-0.198,respectively);Ca was positively correlated with FPG,CSP,and Zn(r=0.163,0.209,and 0.389,respectively);Mg was positively correlated with Zn and Ca(r=0.310 and 0.230,respectively);and Fe was negatively correlated with Zn(r=-0.193).All correlations were statistically significant.Conclusion Patients with T2DM and T2DM complicated with thrombotic diseases exhibit elevated Sestrin2 levels,coagulation dysfunction,and imbalances in trace element.Detecting serum Sestrin2,trace elements,and coagulation indicators may help predict thrombosis and facilitate early intervention in patients with T2DM.
