小儿支气管肺炎不同抗菌药物临床应用及其与病原菌耐药性的关系研究

摘要 目的探究小儿支气管肺炎不同抗菌药物临床应用及其与病原菌耐药性的关系。方法回顾性分析2021年1月至2022年8月开远市人民医院儿童医院收治的50例小儿支气管肺炎患儿的临床资料,统计其抗菌药物用药频度(defined daily doses,DDDs)及其病原菌耐药性情况,分析DDDs与病原菌耐药性的相关性。结果150例患儿痰液样本中,共分离出162株细菌;其中革兰阴性菌检出率为62.96%,革兰阳性菌检出率为37.04%;所有菌株占比前4位由高至低依次为肺炎链球菌(22.84%)、金黄色葡萄球菌(14.20%)、肺炎克雷伯菌(12.35%)、大肠埃希菌(11.73%),耐药率分别为73.33%、66.67%、100.00%、66.67%;肺炎链球菌、金黄色葡萄球菌、肺炎克雷伯菌、大肠埃希菌的DDDs与病原菌耐药率均呈正相关(r>0,P<0.05)。结论小儿支气管肺炎DDDs与病原菌耐药性正相关,需加强儿科抗生素用药管理,提升用药合理性,降低病原菌耐药率。 Objective To explore the clinical application of different antimicrobial drugs in pediatric bronchopneumonia and their relationship with pathogenic bacteria resistance.Methods The data of 50 cases of children's bronchopneumonia treated in Children's Hospital,Kaiyuan People's Hospital from January 2021 to August 2022 were analyzed retrospectively,and the frequency of use of defined daily doses(DDDs)and the drug resistance of pathogenic bacteria were counted,and the correlation between DDDs and drug resistance of pathogenic bacteria was analyzed.Results 162 strains of bacteria were isolated from sputum samples of 150 children;among them,the detection rate of gram-negative bacteria was 62.96%,and the detection rate of gram-positive bacteria was 37.04%;all the strains from high to low were Streptococcus pneumoniae(22.84%),Staphylococcus aureus(14.20%),Klebsiella pneumoniae(12.35%)and Escherichia coli(11.73%),the drug resistance rates were 73.33%,66.67%,100.00%and 66.67%,respectively;the DDDs of Streptococcus pneumoniae,Staphylococcus aureus,Klebsiella pneumoniae and Escherichia coli were positively correlated with the resistance rate of pathogenic bacteria(r>0,P<0.05).Conclusion DDDs of pediatric bronchopneumonia is positively correlate with pathogen resistance,it is necessary to strengthen the management of antibiotic use in children,improve the rationality of drug use and reduce the drug resistance rate of pathogenic bacteria.
