摘要
目的探究基于“温通解结”理论的瑶医油针疗法对退行性膝骨关节炎(degenerative knee osteoarthritis,DKOA)的临床疗效。方法选取2021年8月至2022年8月于百色市中医医院诊治的80例DKOA患者作为研究对象,采用随机数字盲法分为观察组与对照组,各40例。对照组使用双氯芬酸钠双释放肠溶胶囊治疗,观察组采用瑶医千斤拔油针疗法,比较两组血清细胞因子[白细胞介素-1(interleukin-1,IL-1)、白细胞介素-6(interleukin-6,IL-6)和肿瘤坏死因子-α(tumor necrosis factor-α,TNF-α)]、膝关节X线影像学[膝关节Kellgren-Lawrence(KL)分级]、临床疗效、疼痛程度、西安大略大学和麦克马斯特大学骨关节炎指数(Western Ontario and McMaster Universities osteoarthritis index,WOMAC)评分。结果治疗后,两组IL-1、IL-6和TNF-α水平均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);两组膝关节KL分级比较差异无统计学意义;观察组治疗总有效率高于对照组,差异有统计学意义(P<0.05);治疗后,两组视觉模拟评分法评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05);治疗后,两组WOMAC评分均低于治疗前,且观察组低于对照组,差异有统计学意义(P<0.05)。结论基于“温通解结”理论的瑶医油针疗法治疗DKOA疗效显著,可有效缓解患者疼痛,降低膝关节炎症因子水平,改善膝关节功能,且无严重不良反应,值得临床推广应用。
Objective To investigate the clinical efficacy of Yao Medicine Oil-needle therapy for degenerative knee osteoarthritis(DKOA)based on the"warming,unblocking,resolving stasis"theory.Methods 80 patients with DKOA treated in the Baise Traditional Chinese Medicine Hospital from August 2021 to August 2022 were selected as the study subjects,and they were divided into the observation group and the control group using the randomized numerical blind method,with 40 cases in each group.The control group received diclofenac sodium double-release enteric-coated capsules,while the observation group was treated with Yao Medicine Qianjinba Oil-needle therapy.Serum cytokine levels(interleukin-1[IL-1],interleukin-6[IL-6]and tumor necrosis factor-α[TNF-α]),knee joint X-ray imaging(Kellgren-Lawrence[KL]classification),clinical efficacy,pain intensity and Western Ontario and McMaster Universities osteoarthritis index(WOMAC)scores were compared between the two groups.Results After treatment,the levels of IL-1,IL-6 and TNF-αof the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05);there was no statistically significant difference in the knee joint KL classification between the two groups;the total effective rate in the observation group was higher than that in the control group,and the difference was statistically significant(P<0.05);after treatment,the visual analogue scede scores of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05);after treatment,the WOMAC scores of the two groups were lower than those before treatment,and the observation group was lower than the control group,and the differences were statistically significant(P<0.05).Conclusion Yao Medicine Oil-needle therapy based on the"warming,unblocking,resolving stasis"theory has significant clinical efficacy in the treatment of DKOA,it can effectively relieve patient pain,reduce the level of inflammatory factors in the knee joint,and improve knee joint function,without serious adverse reactions,it is worthy of clinical promotion and application.
