摘要
目的:探讨剖宫产术中应用低剂量间羟胺对高龄二次剖宫产产妇并发症及新生儿相关指标的影响。方法:随机数字表法将本院2023年2月-2024年2月收治的132例高龄二次剖宫产产妇分为对照组(n=66)和观察组(n=66)。麻醉开始前20 min观察组泵入间羟胺0.04μg/kg/min、对照组泵入等量生理盐水,均行腰硬联合麻醉。对比两组并发症、新生儿相关指标脐动脉血酸碱度(pH)值、出生1min及5min Apgar评分、二氧化碳分压(PaCO_(2))、氧分压(PaO_(2)),血流动力学指标心率(HR)、平均动脉压(MAP)及术后满意度。结果:观察组产妇并发症总发生率(6.1%)低于对照组(18.2%),新生儿脐动脉血pH值、1min及5min Apgar评分、PaCO_(2)、PaO_(2)均高于对照组;麻醉前(T0)两组HR、MAP比较无差异(P>0.05),术后即刻两组HR、MAP均较麻醉前降低,但观察组高于对照组;产妇术后满意度(95.5%)高于对照组(81.8%)(均P<0.05)。结论:低剂量间羟胺麻醉可以减少高龄二次剖宫产产妇并发症,改善新生儿早期健康状态,稳定产妇血流动力学指标,提高术后满意度。
Objective:To investigate the effects of the low dose metaraminol used during the second cesarean section of the advanced age women on their complications and neonatal related indexes.Methods:132 advanced age women with the second cesarean section in the hospital were divided into control group(n=66)and observation group(n=66)according to the random number table method from February 2023 to February 2024.The women in the observation group were pumped given metaraminol 0.04μg/(kg min)at 20 min before anesthesia,and the women in the control group were injected with the same amount of normal saline,and then the women in the two groups were given the combined epidural anesthesia.The complications rate,the values of the neonatal related indexes,such as the umbilical arterial blood pH(pH),the neonatal Apgar score at 1min and at 5min after birth,the partial pressure of carbon dioxide(PaCO_(2))and the partial pressure of oxygen(PaO_(2)),the values of the hemodynamic indexes,such as heart rate(HR)and mean arterial pressure(MAP),and the postoperative satisfaction of the women were compared between the two groups.Results:The total incidence(6.1%)of the complications of the women in the observation group was significantly lower than that(18.2%)of the women in the control group,and the values of umbilical artery blood pH,Apgar score at lmin and 5min after birth,PaCO_(2)and PaO_(2)of the newborn of the women in the observation group were significantly higher than those of the women in the control group.Before anesthesia(To),there was no significant difference in the values of HR and MAP of the women between the two groups(P>0.05).Immediately after operation,the values of HR and MAP of the women in the two groups had decreased significantly,but which of the women in the observation group were significantly higher than those of the women in the control group.The satisfaction(95.5%)of the women in the control group was significantly higher than that(81.8%)of the women in the control group(all P<0.05).Conclusion:The low dose metaraminol anesthesia can reduce the complications of the advanced age women with the second cesarean section,improve the health status of their newborns,stabilize their hemodynamic status and increase their postoperative satisfaction.
