下颌第二前磨牙根尖屏障术后根管充填材料三维有限元分析

摘要 目的:根尖发育9期下颌第二前磨牙根尖屏障术后,选择5种根管上半段充填材料,分别为玻璃离子、纳米树脂、牙胶、Biodentin、纤维桩。对比哪种材料能更好的保护根管壁及根尖区牙体组织,为临床工作提供参考。方法:使用CBCT,ABAQUS2020等软件,建立根尖孔未闭合的下颌第二前磨牙模型,模拟力的加载,分析应力分布及最大形变。结果:45°加载时纤维桩的充填模型最大位移值最小,垂直加载时Biodentin充填材料模型最大位移值最小,5种材料中Biodentin作为充填材料时牙体所受的最大等效应力是最小的,但纤维桩充填时根管壁及根尖所受的等效应力相较其余4种材料都是最小的。结论:根尖发育9期时,下颌第二前磨牙根管上半段使用纤维桩或Biodentin充填,可减轻根管壁和根尖区受力,降低根折风险。 AIM:To compare five root canal upper segment filling materials(Glass lonomer Cement,Composite Resin,Guttar-Percha,Biodentin,Fiber-Rein Forced Post)after apical barrier surgery for the mandibular second premolar at the apical development stage 9,and identify which material better protects the root canal wall and apical dental tissue,providing a reference for clinical practice.METHODS:Using CBCT,ABAQUS2020,and other software,a model of the mandibular second premolar with an unclosed apical foramen was established to simulate force loading,analyze stress distribution,and maximum deformation.RESULTS:Among these materials,Biodentin exerts the smallest equivalent stress on the tooth when used as a filling material.However,when fiber posts are used for filling,the equivalent stress on the root canal wall and apex is the smallest compared to the other four materials.CONCLUSION:At the apical development stage 9,using fiber posts or Biodentin for the upper segment of the mandibular second premolar root canal can reduce the stress on the root canal wall and apical area,and lower the risk of root fracture.
