经皮椎体后凸成形术(PKP)治疗骨质疏松性脊柱压缩骨折的疗效

摘要 目的 探讨骨质疏松性脊柱压缩骨折患者采用经皮椎体后凸成形术(PKP)治疗的临床效果。方法 选择2023年7月至2024年5月医院收治的骨质疏松性脊柱压缩骨折患者54例,采用随机数表方式分组。对照组(n=27)予经皮椎体成形术(PVP)治疗,观察组(n=27)予PKP治疗。比较两组手术效果。结果 观察组手术时间(38.53±4.69)min、透视次数(18.27±2.88)次、骨水泥注入量(5.22±0.67)ml均多于对照组,但住院时间(4.06±1.03)d短于对照组,P<0.05。术后,观察组椎体前缘、后缘高度均大于对照组,P<0.05。观察组术后的VAS评分(2.55±0.87)分、ODI评分(23.74±2.84)分均低于对照组,P<0.05。观察组骨水泥渗漏率为3.70%(1/27),低于对照组的25.93%(7/27),P<0.05。结论 PKP治疗骨质疏松性脊柱压缩骨折患者的手术时间较PVP较长,操作较复杂,但患者在术后疼痛、腰椎功能改善、骨水泥渗漏风险等方面均具有明显优势。 Objective:To evaluate the clinical efficacy of percutaneous kyphoplasty(PKP)in treating osteoporotic vertebral compression fractures(OVCF).Methods:A total of 54 OVCF patients admitted to the hospital from July 2023 to May 2024 were enrolled and randomly divided into two groups using a random number table.The control group(n=27)underwent percutaneous vertebroplasty(PVP),while the observation group(n=27)received PKP.Surgical outcomes、including operative parameters、radiographic improvements,and complications were compared.Results:In the observation group,the operation time was(38.53±4.69)min,the times of fluoroscopy were(18.27±2.88)times,and the amount of bone cement injected was(5.22±0.67)ml,but the hospital stay was(4.06±1.03)d shorter than that in the control group,P<0.05.After operation,the heights of the anterior and posterior edges of the vertebral body in the observation group were higher than those in the control group,P<0.05.The VAS score(2.55±0.87)and ODI score(23.74±2.84)in the observation group after operation were lower than those in the control group,P<0.05.The leakage rate of bone cement in the observation group was 3.70%(1/27),which was lower than that in the control group 25.93%(7/27),P<0.05.Conclusion:Although PKP requires longer operative time and is technically more demanding than PVP,it provides significant advantages in pain relief,functional recovery,and reduced complication rates(e.g.,cement leakage)for OVCF patients.
