帕金森病患者左旋多巴诱发运动障碍发生情况及相关因素分析

摘要 目的 探究左旋多巴为主要治疗的帕金森病(PD)患者诱发运动障碍的现状及相关影响因素。方法 回顾性分析苏州市立医院2021年3月至2023年3月收治的帕金森患者58例,以单因素、Logistic多因素回归进行左旋多巴诱导的运动障碍(LID)发生情况分析,总结相关影响因素。结果 58例患者中,有12例发生LID,占20.69%;经单因素分析可见,左旋多巴用药时间、剂量及H-Y分级、初始诊疗药物为LID发生的影响因素。将单因素分析结果中P<0.05的因素纳入Logistic多因素回归分析,结果显示,左旋多巴用药疗程(>5年)、每日服用剂量(超过300 mg/d)、H-Y分级(>3.0级)及初始用药(左旋多巴)为LID发生的独立影响因素。结论 PD患者出现LID的概率较高,可结合患者实际情况予以干预,如缩短用药疗程、降低用药剂量、尽早接受规范化诊疗等,以降低LID发生率。 Objective:To explore the current situation and related factors of dyskinesia induced by levodopa in Parkinson's disease(PD).Methods:58 patients with Parkinson's disease admitted to Suzhou Municipal Hospital from March 2021 to March 2023 were retrospectively analyzed.The incidence of levodopa-induced dyskinesia(LID)was analyzed by univariate and Logistic multivariate regression,and the related influencing factors were summarized.Results:Among 58 patients,12 cases developed LID,accounting for 20.69%.Univariate analysis showed that the time、dosage、H-Y classification and initial diagnosis and treatment drugs of levodopa were the influencing factors of LID.The factors with P<0.05 in univariate analysis were included in Logistic multivariate regression analysis.The results showed that levodopa treatment course(>5 years)、daily dosage(over 300 mg/d)、H-Y grade(>3.0)and initial medication(levodopa)were independent risk factors for LID.Conclusion:The probability of LID in PD patients is high,and intervention can be taken according to the actual situation of patients,such as shortening the course of medication,reducing the dosage and receiving standardized diagnosis and treatment as soon as possible,so as to reduce the incidence of LID.
