摘要
本研究通过问卷调查和深度访谈,系统评估了西部地区S县医联体十年建设成效。调研对象涵盖医务人员、患者及政府相关部门,数据分析显示:医联体显著提升了县域医疗服务能力,门诊及住院人次翻倍,患者满意度达88.32%,县域就诊率提升至96.8%,学科建设与医院综合实力大幅增强。然而,发展瓶颈凸显,包括医院负债压力大、政策支持不足、信息化建设滞后、成员间协同机制缺失及医保联动改革不深入等问题。建议通过整合县域医疗资源、强化分级诊疗、优化医保支付方式、推进信息化“一张网”建设、落实政府统筹责任等举措,推动医联体向紧密型医疗集团转型,实现服务、管理、利益一体化,为县域医疗高质量发展提供参考。
This study systematically evaluated the ten-year construction achievements of the medical alliance in S County in the western region through questionnaire surveys and in-depth interviews.The research subjects covered medical staff,patients and the relevant government departments.Data analysis shows that the medical alliance has significantly enhanced the medical service capacity in the county.The number of outpatients and inpatients has doubled;the patient satisfaction rate has reached 88.32%;and the county-level medical treatment rate has increased to 96.8%.The discipline construction and the comprehensive strength of the hospital have been greatly enhanced.However,development bottlenecks have become prominent,including problems such as heavy debt pressure on hospitals,insufficient policy support,lagged information construction,the absence of a collaborative mechanism among members,and the insufficient in-depth reform of medical insurance linkage.Therefore,it is suggested that measures such as integrating county-level medical resources,strengthening hierarchical medical treatment,optimizing medical insurance payment methods,promoting the construction of an informatization-based“one network”,and implementing the government’s overall planning responsibility be taken to promote the transformation of medical alliances into close-knit medical groups,achieve the integration of services,management and interests,and provide references for the high-quality development of the county-level medical care.
