不同频率推拿手法对神经病理性疼痛大鼠脊髓背角突触可塑性的影响

Effects of different frequencies of massage manipulations on the synaptic plasticity in the dorsal horn of the spinal cord in rats with neuropathic pain

摘要 目的 探讨不同频率推拿手法调节神经病理性疼痛大鼠脊髓背角BDNF、SYN1表达及突触可塑性的镇痛作用机制。方法 于术前、术后第1、3、7、14天测定各组大鼠机械性刺激缩足阈值(PMWT)和热痛缩足反应潜伏期(PWTL);术后第14天,采用ELISA法检测IL-1β水平,Western blot 检测脊髓背角BDNF、SYN1 表达情况;TUNEL染色评估脊髓神经元凋亡情况;透射电镜观察脊髓背角突触超微结构。结果 术后第1、3天,与正常组比较,其余各组大鼠PMWT和PWTL均降低(P<0.01);术后第7、14天,低、中、高频率组的PMWT和PWTL均高于模型组(P<0.05或P<0.01);与模型组比较,低、中、高频率组 IL-1β 、凋亡率均降低(P<0.01),以中频率组降低最显著,低、中频率组BDNF降低(P<0.01)、中频率组SYN1升高(P<0.05)、突触活性带变长(P<0.01),高频率组、假推拿组突触致密物厚度增厚(P<0.05),低、中频率组显著增厚(P<0.01),高频率组突触间隙增宽(P<0.05),低、中频率组及假推拿组显著增宽(P<0.01),均以中频率组改善效果最佳。结论 推拿具有镇痛效果,中频率组效果最佳,其机制是通过降低 BDNF,提高 SYN1 的表达,抑制神经元凋亡,促进突触修复与再生,改变突触可塑性。 Objective To investigate the effects of different frequencies of massage manipulations on the expression of BDNF and SYN1 and the analgesic mechanism of synaptic plasticity in the dorsal horn of the spinal cord in rats with neuropathic pain.Methods The levels of paw withdrawal mechanical threshold(PMWT)and paw withdrawal thermal latency(PWTL)were measured before surgery and on the first,third,seventh,and fourth days after surgery.On the 14th day after surgery,the level of IL-1βwas detected by ELISA,and the expression of BDNF and SYN1 in the dorsal horn of the spinal cord was detected by Western blot.TUNEL staining was used to assess the apoptosis of spinal cord neurons,and transmission electron microscopy was used to observe the ultrastructure of synapses in the dorsal horn of the spinal cord.Results On the first and third days after surgery,compared with the control group,the levels of PMWT and PWTL were all decreased in the other groups(P<0.01),and on the 7th and 14th days after operation,the levels of PMWT and PWTL of the low-,medium-and high-frequency groups were higher than those of the model group(P<0.05 or P<0.01).Compared with the model group,the IL-1βand apoptosis rates of the low-,medium-and high-frequency groups were decreased(P<0.01),with a most significant decrease in the medium-frequency group,and the level of BDNF was decreased in the low-and medium-frequency groups(P<0.01),and the expression of SYN1 was increased in the medium-frequency group(P<0.05),and the synaptic active zone became longer(P<0.01),the thickness of synaptic dense matter in the high-frequency group and the pseudo-massage group was thickened(P<0.05),while the low-and medium-frequency groups showed a significant thickening(P<0.01),and the synaptic cleft was widened in the high-frequency group(P<0.05),and the low-and medium-frequency groups and the sham massage group showed a significant widening(P<0.01),with the medium-frequency group showing the best improvement effect.Conclusion Massage has an analgesic effect,with the medium-frequency group showing the best effect,whose mechanisms involve reducing the level of BDNF,increasing the expression of SYN1,inhibiting neuronal apoptosis,promoting synaptic repair and regeneration,and changing synaptic plasticity.
