摘要
国医大师熊继柏传承经典,守正创新,对胸痹有独特认识,并形成特色诊疗思路。其认为胸痹成因是内外合邪,病机乃心脏本虚、包络邪实,临证着眼于心气亏虚、痰瘀痹阻之发病机制。其临床证治胸痹以虚实为纲、寒热为辨、痰瘀为标、气血并调、形神共治;且认为五脏六腑皆令胸痹,非独心也,着重从心与胆、胃、肺、肾、肝、脾关系论治本病,基于心胆同治自拟验方熊氏十味温胆汤对胸痹疗效颇佳。同时,根据熊继柏诊疗经验总结其证治胸痹常用方剂三类:治疗心气亏虚,痰热为主以熊氏十味温胆汤加减,瘀滞为主以颠倒木金散合丹参饮或血府逐瘀汤化裁,痰浊为主以瓜蒌薤白半夏汤、枳实薤白桂枝汤增损。
Professor Xiong Jibai,a master of traditional Chinese medicine,inherits the classics,upholds fundamental principles while makes innovations,develops a unique understanding of chest obstruction,and forms a characteristic diagnosis and treatment idea in this respect.He believes that its etiology features the combination of internal and external pathogens,and its pathogenesis lies in the deficiency of the heart and excessive pathogens in the pericardium.In his clinical syndrome differentiation,Professor Xiong Jibai underscores the pathogenesis of heart qi deficiency,phlegm and blood stasis,while in his clinical treatment,he views the deficiency and excess of heart qi as the root cause,differentiates cold and heat pathogens,takes phlegm and blood stasis as manifestations,regulates qi and blood simultaneously,and treats the body and the spirit together.He also holds that thefive zang-organs and six fu-organs can all cause chest obstruction,not only the heart.Thereby,he argues for treating the disease according to the relationship between the heart and the gallbladder,stomach,lung,kidney,liver and spleen.Based on the stand of simultaneously treating the heart and the gallbladder,he applies the self-prescribed Xiong's Shiwei Wendan Decoction in his treatment of chest obstruction,achieving a significant efficacy.In addition,based on his experience in the diagnosis and treatment of chest obstruction,this study works out the regularities of his three commonly used prescriptions:modified Xiong's Shiwei Wendan Decoction to handle qi deficiency,phlegm heat,modified Diandao Mujin Powder combined with Danshen Decoction or Xuefu Zhuyu Decoction to treat stasis stagnation,modified Gualou Xiebai Banxia Decoction in combination with Zhishi Xiebai Guizhi Decoction to deal with phlegm turbidity.
