摘要
目的探讨老年慢性阻塞性肺疾病(COPD)稳定期应用固本平喘化痰方的效果。方法选择2023年2月至2024年8月南安市医院中西医结合科诊治的80例老年COPD患者,根据随机数字法分为研究组(40例)和对照组(40例)。对照组口服常规西药治疗,研究组予以固本平喘化痰方治疗。分别于治疗前、治疗3个月,对比2组肺通气功能[用力肺活量(FVC)、最大呼气中段流量(MMEF)、静息分钟通气量(MV)、肺泡通气量(VA)]、气道重塑指标[基质金属蛋白酶9(MMP-9)、呼出气一氧化氮(eNO)、半胱氨酸白三烯(Cys-LTs)]、临床症状评分[自我评估量表(CAT)评分、改良版英国医学研究会呼吸问卷(mMRC)评分];对比临床疗效。结果治疗后,2组FVC、MMEF、MV、MV均较治疗前上升,研究组更高(P<0.05);治疗后,2组MMP-9、eNO、Cys-LTs、CAT评分、mMRC评分均较治疗前下降,研究组更低(P<0.05);研究组治疗总有效率(95.0%)比对照组(80.0%)高(χ^(2)=4.11,P=0.042)。结论老年慢性阻塞性肺疾病稳定期中应用固本平喘化痰方治疗,可促进气道重塑,改善COPD症状,减轻肺通气功能损害,调节患者呼吸状态,疗效确切。
Objective To investigate the effect of applying Gubeng Pingchuan Huatan formula of in the stable stage of chronic obstructive pulmonary disease(COPD)in the elderly.Methods Eighty elderly patients with COPD diagnosed and treated in the department of integrative medicine of nan'an city hospital from February 2023 to August 2024 were selected and divided into a study group(40 cases)and a control group(40 cases)according to the randomized bimanual blind selection method.The control group was treated with conventional western medicines orally,and the study group was treated with the formula of consolidating the root,calming asthma and resolving phlegm.Before the treatment and 3 months after the treatment,the two groups were compared in terms of pulmonary ventilation function[forceful lung volume(FVC),maximal expiratory mid-expiratory flow(MMEF),resting minute ventilation(MV),alveolar ventilation(VA)],airway remodeling indexes[matrix metalloproteinase 9(MMP-9),exhaled nitric oxide(eNO),cysteinyl leukotrienes(Cys-LTs)],clinical symptom scores[CAT score,mMRC score];and compare the clinical efficacy.Results After treatment,FVC,MMEF,MV,and MV increased in both groups compared with the pre-treatment period,and were higher in the study group(P<0.05).After treatment,MMP-9,eNO,Cys-LTs,CAT scores,and mMRC scores decreased in both groups compared with the pre-treatment period,and were lower in the study group(P<0.05).And the total effective rate of treatment in the study group(95.0%)was higher than that of the control group(80.0%)(χ^(2)=4.11,P=0.042).Conclusion The application of Guben Pingchuan Huatan formula in the stable stage of chronic obstructive pulmonary disease in the elderly can promote airway remodeling,improve the symptoms of chronic obstructive pulmonary disease,reduce the impairment of pulmonary ventilation function,regulate the respiratory status of patients,and the efficacy is exact.
