摘要
目的旨在评估中医药在初次治疗失败的幽门螺杆菌(Hp)感染相关胃炎患者中的临床疗效。方法将100例因耐药致根除Hp失败的慢性胃炎患者随机分为2组,每组50例。对照组患者口服胶体果胶铋胶囊、雷贝拉唑钠肠溶片、克拉霉素缓释片和阿莫西林胶囊2周治疗后,继续予以雷贝拉唑钠肠溶片和莫沙必利片口服2周。治疗组患者在四联药物治疗基础上,加用“健脾清幽汤”口服及配合耳针治疗,疗程为4周。疗程结束后,分析2组患者的Hp根除率、血清胃蛋白酶原Ⅱ(PGⅡ)水平及临床疗效。结果治疗前2组差异无统计学意义,治疗后治疗组的Hp根除率显著高于对照组(χ^(2)=7.04,P=0.008);治疗组的血清PGⅡ水平较对照组有显著下降(t=8.33,P<0.001);治疗组在缓解腹胀、嗳气、反酸、纳差等症状方面明显优于对照组(P<0.01),而2组在胃脘痛症状改善方面差异无统计学意义(P>0.05);治疗组的总有效率显著高于对照组(χ^(2)=6.55,P<0.001)。结论中医药治疗可提高Hp根除率,改善症状,提升治疗效果,适合临床推广。
Objective To observe the clinical effect of traditional Chinese medicine on Helicobacter pylori(Hp)infection related gastritis with initial treatment failure.Methods One hundred patients with chronic gastritis who had failed to eradicate Hp due to drug resistance were randomly divided into treatment control groupnd control group.The 50 cases in the control group were treated with colloidal bismuth pectin capsules,rabeprazole sodium enteric-coated tablets,clarithromycin sustained-release tablets and amoxicillin capsules for 2 weeks.After 2 weeks,rabeprazole sodium enteric-coated tablets and mosapride tablets were continued for 2 weeks.The 50 cases in the treatment group were treated with modified Jianpi-Qingyou soup combined with auricular acupuncture treatment on the basis of quadruple drug treatment in the control group for 4 weeks.After the course of treatment,the Hp eradication rate,serum PGⅡlevel and clinical efficacy of the two groups were analyzed.Results The eradication rate of Hp in the treatment group was better than that in the control group(χ^(2)=7.04,P=0.008).The serum PGⅡlevel in the treatment group was significantly lower than that in the control group(t=8.33,P<0.001).The treatment group on fullness,belching,acid reflux,anorexia and other symptoms improved significantly better than the control group(P<0.05).There was no significant difference in the improvement of epigastric pain symptoms between the two groups(P>0.05).The total effective rate of the treatment group was significantly better than the control group(χ^(2)=6.55,P<0.001).Conclusion The treatment of traditional Chinese medicine can not only improve the eradication rate of Hp,but also significantly improve the clinical symptoms and improve the clinical treatment effect,which is worthy of clinical application.
