摘要
目的评估使用Ilizarov技术的胫骨延长术结合康复训练在治疗青少年双下肢不等长中的临床疗效。方法将我院收治的双下肢不等长的青少年患者32例行Ilizarov技术并结合康复训练治疗。以Ilizarov应用研究学会(ASAMI)评价标准评定骨愈合及功能恢复情况,分别利用髋关节功能评分标准(Harris)、美国特种医院膝关节评分(HHS)、下肢Fugl-Meyer运动功能得分(FMA-LE)评分标准对患者术后下肢各关节功能的改善状况进行评价,用改良Ashworth评分和Barthel指数对康复训练改善水平进行评价。结果根据ASAMI评价标准,优17例,良13例,中2例,优良率为93.8%。短侧肢体延长长度范围在4.2~6.0 cm,平均5.4 cm,外固定架拆除时间6~15个月,平均9.7月。手术拆除外固定架后,术后患者的Harris评分、HHS评分、FMA-LE评分均较术前有所改善(P<0.05)。末次随访的改良Ashworth评分和Barthel指数较术前有所改善(P<0.05)。结论使用Ilizarov技术的胫骨延长术结合康复训练对于青少年下肢不等长的治疗在功能恢复及提升生活质量方面取得了良好的临床效果。
Objective To evaluate the clinical efficacy of tibial lengthening using the Ilizarov technique combined with rehabilitation training in the treatment of unequal lower limb length(LLD)in adolescents.Methods A total of 32 adolescent patients with unequal lower limb lengths were treated with the Ilizarov technique and rehabilitation training.The Association for the Study and Application of the Method of Ilizarov(ASAMI)evaluation criteria were employed to assess bone healing and functional recovery.Additionally,the Harris Hip Function Score,American Hospital Knee Score(HHS)and Fugl-Meyer Lower Limb Motor Function Score(FMA-LE)were used to evaluate the improvement of joint function in the lower limbs after surgery.The modified Ashworth score and Barthel index were also used to evaluate the level of improvement.Results According to ASAMI evaluation standards,17 cases were classified as excellent,13 as good,and 2 as middle,the good rate was 93.8%.The extension length of the short limb ranged from 4.2 to 6.0 cm,with a mean of 5.4 cm.The time of external fixator removal varied from 6 to 15 months,with a mean of 9.7 months.After external fixator removal,the Harris Hip Function Score,HHS Score and FMA-LE Score improved compared to those before surgery(P<0.05).Furthermore,the modified Ashworth score and Barthel index at the final follow-up showed significant improvement compared to preoperative values(P<0.05).Conclusion Tibial lengthening using the Ilizarov technique combined with rehabilitation training has good effect in the treatment of unequal lower limb lengths in adolescents,which can improve functional recovery and quality of life.
