摘要
目的观察合募配穴电针对活动期溃疡性结肠炎临床症状、肠道菌群、血清炎症因子和肠黏膜屏障功能以及患者生活质量的影响。方法将94例活动期溃疡性结肠炎患者进行前瞻性随机对照研究,采用随机数字表法分为观察组和对照组,每组47例。对照组给予西药治疗,观察组在对照组基础上另予合募配穴电针治疗。观察两组患者治疗前后Sutherland疾病活动指数、Mayo内镜评分、炎症性肠病生存质量问卷评分、肠道菌群、血清肠黏膜屏障功能(D-乳酸、二胺氧化酶和低氧诱导因子-1α)水平、血清炎症因子(干扰素-γ、Toll样受体4和单核细胞趋化蛋白-1)水平的变化。结果治疗后,两组Sutherland疾病活动指数、Mayo内镜评分、肠球菌数量、血清肠黏膜屏障功能和炎症因子水平均较治疗前降低(P<0.05),两组双歧杆菌和乳酸杆菌的数量以及炎症性肠病生存质量问卷评分均较治疗前升高(P<0.05);且观察组上述指标均优于对照组(P<0.05)。结论在西药治疗基础上,合募配穴电针治疗可有效改善活动期溃疡性结肠炎的症状,调节肠道菌群、血清肠黏膜屏障功能及血清炎症因子水平,提高患者生活质量。
Objective To investigate the clinical effect of electroacupuncture at He-acupoint and Mu-acupoint combinations on clinical symptoms,gut microbiota,serum inflammatory factors,intestinal mucosal barrier function,and quality of life in patients with active ulcerative colitis(UC).Method A prospective randomized controlled trial was conducted on 94 patients with active UC.They were divided into an observation group and a control group,with 47 cases in each group using the random number table method.The control group received conventional Western medication,while the observation group received additional electroacupuncture at He-acupoint and Mu-acupoint combinations based on the control regimen.Changes in Sutherland disease activity index,Mayo endoscopic score,inflammatory bowel disease questionnaire(IBDQ)score,gut microbiota composition,serum markers of intestinal mucosal barrier function(D-lactic acid,diamine oxidase,hypoxia-inducible factor-1α),and inflammatory cytokines(interferon-γ,toll-like receptor 4,monocyte chemoattractant protein-1)were evaluated before and after treatment in both groups.Result After treatment,both groups showed significant reductions in Sutherland index,Mayo endoscopic score,Enterococcus counts,serum markers of mucosal barrier impairment,and inflammatory cytokine levels(P<0.05),while the counts of Bifidobacterium and Lactobacillus,as well as IBDQ scores,increased significantly(P<0.05).The observation group showed more significant improvements across all above parameters compared with the control group(P<0.05).Conclusion Electroacupuncture at He-acupoint and Mu-acupoint combinations,when added to standard Western medication,can effectively alleviate symptoms of active UC,modulate intestinal microbiota,improve serum markers of mucosal barrier and inflammation,and enhance patients’quality of life.
