摘要
目的分析改良后的环抱技术修复Maisonneuve骨折合并下胫腓韧带联合分离的临床效果。方法纳入2019年5月至2022年5月期间采用改良环抱技术治疗的Maisonneuve骨折患者15例。与之前的技术类似,应用了两根带有NICE结的双缝线,并在矢状面添加了额外的缝合线固定。美国足踝外科协会(American Orthopedic Foot and Ankle Society,AOFAS)踝-后足功能评分、Olerud和Molander评分以及视觉模拟评分法(visual analogue scale,VAS)评分用于评估术后功能。采用术前及术后X线片和三维CT进行客观评估。采用独立t检验来比较所有术前和术后数据。结果患者平均年龄为(28.1±13.0)岁,随访时间为(16.4±3.7)个月。AOFAS、Olerud和Molander以及VAS的平均分分别为(95.2±6.2)分、(96.7±4.5)分和(0.5±0.9)分。CT结果显示,与正常侧相比,受伤踝关节术后CT评估结果无差异。同样,所有CT数据显示术前和术后数据之间存在显著差异。结论改良环抱技术固定Maisonneuve骨折合并下胫腓联合损伤是一种有效、安全、可重复的方法。
Objective To analysis the clinical effectiveness of the modified embrace technique on the treatment of diastasis of syndesmosis in Maisonneuve fracture.Methods 15 Maisonneuve fracture with diastasis syndesmosis patients treated with modified embrace technique between May 2019 to May 2022 were included.Similar to the previous technique,two double sutures with NICE knots were applied and additional suture fixation was added in the sagittal plane.The American Orthopedic Foot and Ankle Society(AOFAS)ankle-hindfoot score,the Olerud and Molander score,and the visual analogue scale(VAS)were used to evaluate the function of the hindfoot.Preoperative and postoperative radiographs and three-dimensional CT were used for objective evaluation.Independent t-tests were used to compare all preoperative and postoperative data.Results The mean age of the patients was(28.1±13.0)years,and the follow-up time was(16.4±3.7)months.The mean scores of AOFAS,Olerud and Molander scores,and VAS were(95.2±6.2),(96.7±4.5),and(0.5±0.9),respectively.CT results showed no difference in the postoperative data of the injured side,compared with the normal side.Besides,all CT data showed significant differences between the preoperative and postoperative data.Conclusion The modified embracing technique is an effective,safe,and reproducible method for the fixation of Maisonneuve fractures combined with tibiofibular syndesmotic injured.
