TDCS对学龄前低功能孤独症谱系障碍儿童脑功能静息态调节效应的研究

摘要 目的:探究经颅直流电刺激(tDCS)对学龄前低功能孤独症谱系障碍(ASD)儿童脑功能静息态调节效应的影响。方法:从佛山市南海区妇幼保健院儿童康复科收集2022年1月至2024年10月期间就诊的学龄前低功能ASD患儿60例,按照就诊时间先后顺序均分为两组,对照组(n=30)与观察组(n=30)。观察组给予tDCS治疗,对照组给予tDCS伪刺激治疗。比较两组治疗前后孤独症行为量表(ABC)评分、孤独症治疗评估量表(ATEC)评分;比较治疗后额叶(FR)、顶叶(PA)、枕叶(OC)、左颞叶(LT)、右颞叶(RT)5个脑区的小波熵和样本熵。结果:与治疗前相比,两组ABC评分、ATEC评分治疗后均下降,且观察组评分均低于对照组(P<0.05);观察组治疗后-治疗前差值较对照组大(P<0.05);经过治疗后,观察组PA、OC、LT、RT四个区域的小波熵、样本熵数据变化较对照组的数据变化小(P<0.05)。结论:tDCS调节干预可积极改善学龄前低功能ASD患儿的ASD行为,并对改善患儿的症状有极大的帮助,值得推广研究。 Objective:To explore the effects of transcranial direct current stimulation(tDCS)on resting state regulation of brain function in preschool children with low functioning autism spectrum disorder(ASD).Methods:A total of 60 preschool children with low-functioning ASD were collected from the Children’s Rehabilitation Department of Nanhai Maternal and Child Health Hospital of Foshan City from January 2022 to October 2024,and were divided into two groups according to the order of treatment time:control group(n=30)and observation group(n=30).The observation group was treated with tDCS,and the control group was treated with tDCS pseudo-stimulation.The scores of autism Behavior Scale(ABC)and Autism Treatment Assessment Scale(ATEC)were compared between the two groups before and after treatment.The wavelet entropy and sample entropy of 5 brain regions after treatment were compared in frontal lobe(FR),parietal lobe(PA),occipital lobe(OC),left temporal lobe(LT)and right temporal lobe(RT).Results:Compared before treatment,the ABC scores and ATEC scores after treatment were decreased in the two groups,and the scores in the observation group were lower than the control group(P<0.05).The difference between after treatment and before treatment in the observation group was larger than that in the control group(P<0.05).After treatment,the changes of wavelet entropy and sample entropy data of PA,OC,LT and RT in the observation group were smaller than those in the control group(P<0.05).Conclusion:tDCS regulation intervention can actively improve the ASD behavior of preschool children with low functioning ASD,and has great help to improve the symptoms of children,which is worth promoting research.
