摘要
目的 探讨计划-实施-检查-处理(plan-do-checkact,PDCA)循环管理在手术室器械包装物品合格率中的应用价值。方法 选取2022年1—12月在厦门大学附属妇女儿童医院/厦门市妇幼保健院实施PDCA循环管理模式前的12 423件手术室器械纳入实施前,将2023年1—12月间实施PDCA循环管理模式后的13 732件手术室器械纳入实施后,在实施传统经验管理和实施PDCA循环管理后比较消毒供应室工作人员知识知晓、操作技能情况。比较实施前后手术室器械包装物品合格率,由质控小组评估手术室器械清洗、消毒及包装质量情况,并采用供应室自制的满意度调查问卷评估手术室医护人员对供应室手术器械质量管理满意程度。结果 实施后,工作人员知识知晓率(100%)、操作正确率(100%)均高于实施前(64.71%、58.82%),差异有统计学意义(P <0.05)。实施后,手术器械包装闭合完好率(99.65%)、包内器械完整率(99.90%)、包内器械功能完好率(99.95%)、包内器械种类齐全率(99.99%)、灭菌包标识正确率(99.97%)均分别高于实施前(98.67%、99.56%、99.69%、99.82%、99.89%),差异有统计学意义(P <0.05)。实施后,手术室器械清洗、消毒、包装质量评分高于实施前,差异有统计学意义(P <0.05)。实施后,医护人员对供应室手术器械质量管理总满意率(100%)高于实施前(80.00%),差异有统计学意义(P <0.05)。结论PDCA循环管理应用于手术室器械管理过程,有利于消毒供应室工作人员加深对手术室器械相关知识认知,熟练操作技能,进而提高手术室器械包装物品合格率,提升手术室器械管理质量,有利于提升各科室医护人员对手术室器械管理满意度。
Objective To explore plan-do-check-act(PDCA)cycle management in the qualification rate of surgical instrument packaging items.Methods A total of 12423 operating room instruments before implementing the PDCA cycle management model at Women and Children′s Hospital,School of Medicine,Xiamen University from January 2022 to December 2022 were included before implementation,and 13732 operating room instruments after implementing the PDCA cycle management model from January 2023 to December 2023 were included after implementation.After implementing traditional experience management and PDCA cycle management,the knowledge and operational skills of disinfection supply room staff were compared,and the qualification rate of operating room instrument packaging before and after implementation was compared.Compare the qualification rate of surgical instrument packaging before and after implementation,evaluate the cleaning,disinfection,and packaging quality of surgical instruments by the quality control team,and use a satisfaction survey questionnaire self-made by the supply room to evaluate the satisfaction level of medical staff in the operating room with the quality management of surgical instruments in the supply room.Results After implementation,the knowledge awareness rate(100%)and operational accuracy rate(100%)of the staff were higher than before implementation(64.71%,76.47%),and the differences were statistically significant(P<0.05).After implementation,the closure integrity rate of surgical instrument packaging(99.65%),the integrity rate of instruments inside the package(99.90%),the functional integrity rate of instruments inside the package(99.95%),the completeness rate of instrument types inside the package(99.99%),and the accuracy rate of sterilization package labeling(99.97%)were all higher than before implementation(98.67%,99.56%,99.69%,99.82%,99.89%),and the differences were statistically significant(P<0.05).After implementation,the scores for instrument cleaning,disinfection,and packaging quality in the operating room were higher than before implementation,and the difference was statistically significant(P<0.05).The overall satisfaction rate of medical staff with the quality management of surgical instruments in the supply room after implementation(100%)was higher than before implementation(86.67%),and the difference was statistically significant(P<0.05).Conclusion The application of PDCA cycle management in the process of operating room instrument management is beneficial for disinfection supply room staff to deepen their understanding of operating room instrument related knowledge,proficiently operate skills,thereby improving the qualification rate of operating room instrument packaging items,enhancing the quality of operating room instrument management,and promoting the satisfaction of medical staff in various departments with operating room instrument management.
