摘要
目的:探讨脊柱内固定术对脊柱骨折合并脊髓损伤患者脊柱稳定性与脊髓功能恢复的影响.方法:选取2022年2月至2024年1月本院收治的102例脊柱骨折合并脊髓损伤患者作为研究对象.依照手术方案,将其分为观察组(n=51,接受经皮微创脊柱内固定术)和对照组(n=51,接受后路切开复位椎弓根螺钉内固定术).比较两组患者手术相关指标、脊柱稳定性、脊髓功能恢复情况以及并发症.结果:观察组患者术中出血量少于对照组,手术时间、切口长度与术后住院时间均短于对照组,差异均有统计学意义(P<0.05).术后3个月、6个月,观察组患者伤椎椎体前缘高度比高于对照组,后凸Cobb角小于对照组,差异均有统计学意义(P<0.05).术后6个月,观察组患者美国脊髓损伤协会(ASIA)分级优良率高于对照组,并发症总发生率低于对照组,差异均有统计学意义(P<0.05).结论:经皮微创脊柱内固定术能更有效地促进脊柱骨折合并脊髓损伤患者脊柱稳定性与脊髓功能的恢复,且安全性较高.
Objective:To explore the effect of spinal fixation on spinal stability and spinal cord function recovery in patients with spinal fracture combined with spinal cord injury.Methods:A total of 102 patients with spinal fracture and spinal cord injury who were treated in our hospital from February 2022 to February 2024 were selected as research subjects.According to the surgical plan,they were divided into observation group(n=51,received percutaneous minimally invasive spinal internal fixation)and control group(n=51,received posterior open reduction and pedicle screw internal fixation).The operation-related indexes,spinal stability,recovery of spinal cord function and complications were compared between the two groups.Results:The intraoperative blood loss in observation group was lower than that in control group,and the operation time,incision length and postoperative hospital stay were shorter than those in control group,and the differences were statistically significant(P<0.05).At 3 and 6 months after treatment,the anterior height ratio of the injured vertebral body in observation group was higher than that in control group,and the Cobb angle of kyphosis was smaller than that in control group,and the differences were statistically significant(P<0.05).At 6 months after operation,the excellent and good rate of American spinal cord injury association(ASIA)ranking in observation group was higher than that in control group,and the total incidence of complications was lower than that in control group,and the differences were statistically significant(P<0.05).Conclusion:Percutaneous minimally invasive spinal fixation can more effectively restore the spinal stability and promote the recovery of spinal cord function of patients with spinal fracture combined with spinal cord injury,and which is safe.
